Description
Overview of the medicine
Pantoprazole is a proton pump inhibitor (PPI) that decreases the amount of acid produced in the stomach. It is used to treat conditions such as heartburn, gastroesophageal reflux disease (GERD), stomach ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD/Erosive Esophagitis: 40 mg once daily for up to 8 weeks. For Duodenal Ulcer: 40 mg once daily for 2-4 weeks. For Gastric Ulcer: 40 mg once daily for 4-8 weeks. For Zollinger-Ellison Syndrome: Initial dose 40 mg twice daily, adjusted as needed.
Elderly
No dosage adjustment is generally required for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment or those on hemodialysis.
How to Take
Take the tablet whole, do not chew or crush it. It should be taken once daily in the morning, usually 30-60 minutes before breakfast.
Mechanism of Action
Pantoprazole irreversibly blocks the hydrogen/potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, also known as the proton pump, in the gastric parietal cells. This action inhibits the final step in gastric acid secretion.
Pharmacokinetics
Onset
Onset of action within 2.5 hours.
Excretion
Primarily excreted in the urine (approximately 71%) and to a lesser extent in feces (approximately 18%) via biliary excretion.
Half life
Approximately 1 hour (terminal elimination half-life), but duration of acid suppression is much longer.
Absorption
Rapidly absorbed after oral administration; bioavailability is approximately 77%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to pantoprazole or any excipients in the formulation.
- •Patients with severe liver disease may require dose adjustment or careful monitoring.
Drug Interactions
Clopidogrel
Concomitant use may reduce the antiplatelet effect of clopidogrel.
Methotrexate
Increased and prolonged elevation of methotrexate and/or its metabolite serum levels.
Atazanavir, Nelfinavir
Reduced absorption due to pH changes, leading to decreased efficacy of antivirals.
Iron salts, Ketoconazole, Itraconazole
Reduced absorption of drugs that require acidic gastric pH for absorption.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no known specific antidote. In cases of overdose, treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy: Use only if clearly needed and the potential benefits outweigh the potential risks. Lactation: Pantoprazole is excreted in human milk; caution should be exercised.
