Overview of the medicine
Pantoprazole is a proton pump inhibitor (PPI) used to reduce stomach acid production. It is prescribed for conditions like gastroesophageal reflux disease (GERD), peptic ulcer disease, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily, or 40 mg once daily for more severe conditions, usually for 4-8 weeks. For H. pylori eradication, 20 mg twice daily with antibiotics.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function or those on hemodialysis.
How to Take
Take orally, usually once daily in the morning, before a meal. Do not crush, chew, or break the tablet; swallow it whole.
Mechanism of Action
Pantoprazole is a proton pump inhibitor that irreversibly binds to and inhibits the H+/K+-ATPase enzyme system (the 'proton pump') in gastric parietal cells, thereby suppressing gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour (acid suppression), full effect in 2-3 days.
Excretion
Mainly excreted renally (approximately 71%) and a smaller portion via biliary excretion (approximately 18%).
Half life
Approximately 1 hour (terminal half-life), but acid suppression lasts much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 2-2.5 hours.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with rilpivirine-containing products.
Drug Interactions
Warfarin
Potential for increased INR and bleeding.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased plasma concentration of methotrexate.
Iron salts, Ketoconazole, Itraconazole
