Description
Overview of the medicine
Pantoprazole is a proton pump inhibitor (PPI) used to treat certain stomach and esophagus problems (such as acid reflux, ulcers). It works by decreasing the amount of acid your stomach makes. It helps heal acid damage to the stomach and esophagus, helps prevent ulcers, and may help prevent cancer of the esophagus.
Uses & Indications
Dosage
Adults
GERD: 40 mg once daily for up to 8 weeks. Erosive Esophagitis: 40 mg once daily for up to 8 weeks, maintenance 20-40 mg daily. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted as needed.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with hepatic impairment.
Renal_impairment
No dose adjustment necessary.
How to Take
Take orally, once daily, usually in the morning before breakfast. Swallow whole; do not crush, chew, or split the tablet.
Mechanism of Action
Pantoprazole is a proton pump inhibitor that suppresses the final step in gastric acid production by covalently binding to the H+/K+-ATPase enzyme system at the secretory surface of the gastric parietal cell. This binding leads to inhibition of both basal and stimulated gastric acid secretion.

Onset
Within 2.5 hours
Excretion
Mainly excreted renally (approximately 71%) and partly through biliary excretion/feces (approximately 18%).
Half life
Approximately 1 hour (terminal half-life), but duration of acid suppression is much longer.
Absorption
Rapidly absorbed after oral administration; bioavailability is about 77%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.

Contraindications
- Hypersensitivity to pantoprazole or any component of the formulation
- Known hypersensitivity to substituted benzimidazoles
Drug Interactions
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Possible increased and prolonged serum levels of methotrexate.
Atazanavir, Nelfinavir
Reduced plasma concentrations and therapeutic effect of antiretroviral drugs.
Iron Salts, Vitamin B12
Decreased absorption due to reduced gastric acidity.
Storage
Store below 30°C in a dry place, away from light and moisture.
Overdose
There is no known specific antidote. Treatment should be symptomatic and supportive.
