Description
Overview of the medicine
Panzer 40 mg Tablet contains Pantoprazole, which is a proton pump inhibitor (PPI). It reduces the amount of acid your stomach makes, providing relief from heartburn, acid reflux, and helping to heal acid-related damage to the stomach and esophagus.
Uses & Indications
Dosage
Adults
For GERD and erosive esophagitis: 40 mg once daily for up to 8 weeks. For peptic ulcer disease: 40 mg once daily for 2-4 weeks. For Zollinger-Ellison Syndrome: Initial dose of 40 mg twice daily, adjusted as needed.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Panzer 40 mg tablets should be swallowed whole with water, preferably 30-60 minutes before a meal. Do not crush or chew the tablet.
Mechanism of Action
Pantoprazole selectively blocks the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cell, thereby inhibiting the final step in gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression effect occurs within 2.5 hours.
Excretion
Excreted mainly via the kidneys (approximately 80%) and the remainder via the feces.
Half life
Approximately 1 hour (terminal elimination half-life), but the duration of action is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after oral administration; bioavailability is approximately 77%. Peak plasma concentrations are reached in about 2-2.5 hours.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.

Contraindications
- Hypersensitivity to pantoprazole or any component of the formulation.
- Co-administration with atazanavir or nelfinavir due to significant reduction in their plasma concentrations.
Drug Interactions
Warfarin
Increased INR and prothrombin time, monitor carefully.
Methotrexate
May increase methotrexate levels, especially with high doses.
Atazanavir/Nelfinavir
Reduced efficacy due to decreased absorption; avoid co-administration.
