Overview of the medicine
Pizofen TS contains Ketotifen Fumarate, an anti-allergic agent primarily used for the long-term prophylactic treatment of bronchial asthma, allergic bronchitis, and allergic rhinitis.
Uses & Indications
Dosage
Adults
1 mg twice daily (morning and evening) with food. May be increased to 2 mg twice daily in severe cases.
Elderly
No specific dosage adjustment usually required, but caution in patients with impaired renal or hepatic function.
Renal_impairment
Caution and dosage adjustment may be needed based on clinical response and tolerability.
How to Take
To be taken orally, with food, usually in the morning and evening.
Mechanism of Action
Ketotifen is a non-competitive H1-antihistamine and a mast cell stabilizer. It inhibits the release of inflammatory mediators (e.g., histamine, leukotrienes) from mast cells, thereby preventing allergic reactions. It also has antianaphylactic properties and inhibits the accumulation of eosinophils triggered by platelet activating factor (PAF).
Pharmacokinetics
Onset
Antihistaminic effects within 1-2 hours; therapeutic effect for asthma prophylaxis may take several weeks.
Excretion
Primarily excreted via urine (about 60-70%) as metabolites, with a small portion unchanged.
Half life
Plasma elimination half-life is about 12 hours for the parent compound and 21 hours for the active metabolite.
Absorption
Well absorbed from the GI tract (about 60%), but bioavailability is about 50% due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver, mainly by N-glucuronidation.
Side Effects
Contraindications
- •Hypersensitivity to ketotifen or any component of the formulation
- •Patients with epilepsy or a history of seizures (may lower seizure threshold)
Drug Interactions
Oral antidiabetics
Rarely, a reversible decrease in platelet count has been observed, so avoid concomitant use.
CNS depressants (e.g., sedatives, hypnotics, alcohol)
Increased sedation.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, confusion, nystagmus, headache, dizziness, bradycardia, and sometimes coma. Management is symptomatic and supportive, including gastric lavage and activated charcoal if appropriate.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as it may be excreted in breast milk.
