Description
Overview of the medicine
Ponaxen 15 mg tablet contains Xenoprofen, a hypothetical non-steroidal anti-inflammatory drug (NSAID). It is primarily used for the relief of mild to moderate pain, inflammation, and fever. It exerts its effects by reducing the production of certain natural substances in the body that cause inflammation and pain.
Uses & Indications
Dosage
Adults
The usual adult dose is 15 mg orally, 2 to 3 times daily, preferably with food. The maximum daily dose should not exceed 45 mg.
Elderly
Elderly patients may require lower doses due to increased sensitivity and potential for renal impairment. Start with 15 mg once or twice daily and monitor for adverse effects.
Renal_impairment
Dosage adjustment is necessary in patients with mild to moderate renal impairment. Avoid use in severe renal impairment. Consult a physician for specific dosing guidelines.
How to Take
Take Ponaxen tablets orally with a full glass of water, preferably with food or milk to minimize gastrointestinal discomfort. Do not crush, chew, or break the tablet; swallow it whole.
Mechanism of Action
Xenoprofen, like other NSAIDs, acts by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes. This inhibition leads to a reduced synthesis of prostaglandins, which are key mediators of pain, inflammation, and fever. By blocking prostaglandin production, it reduces these symptoms.
Pharmacokinetics
Onset
Analgesic effects typically begin within 30-60 minutes.
Excretion
The majority of the drug and its metabolites are excreted in the urine (about 60-70%), with a smaller portion excreted in the feces (about 20-30%).
Half life
The plasma elimination half-life is approximately 2 to 4 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are typically achieved within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by oxidative processes involving cytochrome P450 enzymes, to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Xenoprofen, aspirin, or other NSAIDs (e.g., asthma, urticaria, allergic-type reactions)
- •Active gastrointestinal bleeding or peptic ulcer disease
- •Severe renal or hepatic impairment
- •Severe heart failure
- •Third trimester of pregnancy
- •Coronary artery bypass graft (CABG) surgery
Drug Interactions
Lithium
May increase plasma lithium levels, leading to lithium toxicity.
Methotrexate
May increase methotrexate plasma levels and toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
May reduce the hypotensive effect of these drugs and increase the risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding. Close monitoring of INR is recommended.
Selective Serotonin Reuptake Inhibitors (SSRIs)
Increased risk of gastrointestinal bleeding.
Storage
Store in a cool, dry place below 30°C (86°F), protected from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric pain, lethargy, dizziness, drowsiness, tinnitus. In severe cases, gastrointestinal bleeding, acute renal failure, coma, and convulsions may occur. Management involves symptomatic and supportive care. Gastric lavage and/or activated charcoal may be considered if ingestion was recent. Monitor vital signs and renal function.
Pregnancy & Lactation
Pregnancy Category C in the first two trimesters and D in the third trimester. Avoid use during the third trimester of pregnancy due to potential adverse effects on the fetal cardiovascular system (premature closure of ductus arteriosus). Use with caution during lactation; small amounts of NSAIDs may be excreted in breast milk. Consult a physician before use.
