Description
Overview of the medicine
Povidone-Iodine is a broad-spectrum antiseptic used for disinfection of skin, wounds, and mucous membranes. It is effective against bacteria, viruses, fungi, and spores.
Uses & Indications
Dosage
Adults
Apply undiluted Povicid solution to the affected skin or wound area as needed, usually 1-3 times daily. Allow to dry. For surgical scrubbing, use according to standard aseptic procedures.
Elderly
No special dosage adjustment is generally needed for elderly patients. However, caution should be exercised in those with pre-existing thyroid conditions.
Renal_impairment
Topical application on intact skin is generally safe. For prolonged use on large areas or broken skin in patients with renal impairment, systemic iodine absorption should be monitored, and thyroid function should be checked.
How to Take
For external use only. Apply directly to the skin or wound. Do not ingest. Avoid contact with eyes, ears, and nostrils. If accidental contact occurs, rinse thoroughly with water.
Mechanism of Action
Povidone-Iodine releases free iodine when in contact with skin or mucous membranes. This free iodine oxidizes microbial cell components, leading to their inactivation and death. It acts non-selectively and quickly.
Pharmacokinetics
Onset
Immediate antiseptic action upon application.
Excretion
Primarily renal excretion for any systemically absorbed iodine.
Half life
Not significantly relevant for topical use; systemically absorbed iodine is excreted renally with a half-life of several hours to days.
Absorption
Minimal systemic absorption occurs from intact skin. However, absorption can be significant from large areas of broken skin, burns, or mucous membranes.
Metabolism
Systemically absorbed iodine is not extensively metabolized; it is utilized by the thyroid gland or excreted.
Side Effects
Contraindications
- Known hypersensitivity to iodine or povidone.
- Thyroid disorders (e.g., hyperthyroidism, goiter, Hashimoto's thyroiditis).
- Before and after radioiodine therapy.
- Newborns and infants, especially premature infants, due to the risk of systemic iodine absorption and thyroid dysfunction.
- Renal failure (for large area application due to potential for systemic absorption).
Drug Interactions
Lithium
Concurrent use with lithium may increase thyroid-suppressing effects, potentially leading to hypothyroidism.
Taurolidine
May lead to toxic products (iodine compounds) when used simultaneously.
Hydrogen Peroxide
Hydrogen peroxide, octenidine, and silver preparations may decrease the antiseptic efficacy of Povidone-Iodine.
Storage
Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Systemic overdose is rare with proper topical use. In cases of accidental ingestion or extensive application on large, broken skin, symptoms may include gastrointestinal irritation (nausea, vomiting, abdominal pain), metabolic acidosis, hypernatremia, and renal impairment. Management is symptomatic and supportive, including gastric lavage if ingested, and monitoring of electrolytes and thyroid function.
Pregnancy & Lactation
Use with caution and only if clearly needed during pregnancy and lactation. Avoid prolonged use on large skin areas, as absorbed iodine may affect fetal/infant thyroid function. Consult a doctor or pharmacist.

YesClinical Trials
Povidone-Iodine has been extensively studied and used in clinical practice for over 60 years. Numerous clinical trials and studies support its efficacy as a broad-spectrum antiseptic for various indications, including surgical preparation, wound care, and mucositis prevention.
Lab Monitoring
- Thyroid function tests (TSH, T3, T4) should be considered for patients undergoing prolonged or extensive topical use, especially in those with risk factors for thyroid disease or neonates.
- Renal function tests may be considered in cases of massive systemic absorption.
Doctor Notes
- Educate patients on the proper application technique and emphasize that it is for external use only.
- Caution should be exercised in patients with pre-existing thyroid disorders; monitor thyroid function with prolonged or extensive use.
- Be aware of potential drug interactions, especially with lithium and hydrogen peroxide.
- Not recommended for routine use in newborns and premature infants due to the risk of iodine-induced hypothyroidism.
Patient Guidelines
- For external use only. Do not swallow.
- Avoid contact with eyes. In case of accidental contact, rinse thoroughly with water.
- Do not use on large areas of broken skin or deep puncture wounds without medical advice.
- Discontinue use if irritation, redness, or swelling occurs.
- Keep out of reach of children.
Missed Dose Advice
Povicid is used on an 'as needed' basis. If a scheduled application is missed, apply it as soon as remembered if still indicated. Do not apply a double amount to make up for a missed application.
Driving Precautions
Povicid is for topical use and is not expected to affect the ability to drive or operate machinery.
