Description
Overview of the medicine
Povidone-iodine 10% w/v solution is a broad-spectrum antiseptic used for disinfecting skin, mucous membranes, and wounds before and after surgical procedures or minor cuts, burns, and abrasions. It helps prevent and treat skin infections.
Uses & Indications
Dosage
Adults
Apply undiluted solution to the affected area as needed, typically once or twice daily, or as directed by a healthcare professional. Allow to air dry.
Elderly
No specific dosage adjustment required; use as per adult dosage.
Renal_impairment
No specific dosage adjustment required as systemic absorption is minimal. Use with caution on large areas due to potential iodine absorption.
How to Take
For external use only. Apply a small amount of solution to the affected area with a sterile cotton swab or gauze. Allow to dry. Do not bandage until dry.
Mechanism of Action
Povidone-iodine releases free iodine when applied to the skin. Free iodine rapidly penetrates the cell walls of microorganisms, disrupting their protein and nucleic acid structure, leading to cell death. It has broad-spectrum antimicrobial activity against bacteria, fungi, viruses, protozoa, and spores.
Pharmacokinetics
Onset
Rapid; bactericidal effect within 30 seconds to 2 minutes.
Excretion
Primarily renal excretion for absorbed iodide. Topically, it's washed off.
Half life
Systemically absorbed iodine has a variable half-life, typically several hours, depending on thyroid function and renal clearance.
Absorption
Minimal systemic absorption through intact skin. Absorption may increase with large surface areas, broken skin, or prolonged contact. Absorbed iodine is metabolized in the body.
Metabolism
Absorbed iodide is largely metabolized by the thyroid gland and excreted.
Side Effects
Contraindications
- •Known hypersensitivity to iodine or povidone
- •Thyroid disorders (e.g., hyperthyroidism, goitre) due to potential iodine absorption
- •Before and after radioiodine therapy
- •Newborns and infants (especially premature infants) due to risk of hypothyroidism
- •Chronic kidney disease (if used extensively)
Drug Interactions
Octenidine
Use concurrently may cause skin discoloration.
Hydrogen peroxide
May reduce the efficacy of both agents.
Lithium (systemic absorption)
May exacerbate hypothyroidism in predisposed individuals.
Silver sulfadiazine, other heavy metal compounds
May form insoluble complexes, reducing efficacy.
Storage
Store at room temperature (below 30°C). Keep away from direct sunlight and heat. Keep bottle tightly closed.
Overdose
Excessive topical application, especially on large areas or broken skin, may lead to iodine toxicity, including metabolic acidosis, renal failure, and thyroid dysfunction. Management involves symptomatic and supportive care. Discontinue use and rinse thoroughly with water.
Pregnancy & Lactation
Use with caution and only if clearly needed. Excessive absorption of iodine may affect fetal thyroid function. Avoid extensive use or prolonged application during pregnancy and lactation. Consult a doctor.
