Description
Overview of the medicine
Povidone-iodine 5% W/W Ointment is a broad-spectrum antiseptic used for the prevention and treatment of skin infections, minor cuts, wounds, and burns. It works by releasing iodine, which kills bacteria, fungi, viruses, and protozoa.
Uses & Indications
Dosage
Adults
Apply a thin layer of Povidone-iodine 5% W/W Ointment to the affected area 1 to 2 times daily. The area may be covered with a sterile dressing.
Elderly
No specific dosage adjustment is required for elderly patients, but caution is advised if extensive application is needed due to potential for systemic absorption.
Renal_impairment
Use with caution in patients with renal impairment, especially if applied to large areas, due to potential for increased systemic iodine absorption and accumulation.
How to Take
For external use only. Clean the affected area thoroughly before applying the ointment. Apply a small amount and gently spread a thin, even layer over the skin. Do not swallow.
Mechanism of Action
Povidone-iodine releases free iodine, which rapidly penetrates microbial cells. It oxidizes proteins, nucleotides, and fatty acids, leading to cellular disruption and death. It has a broad spectrum of microbicidal activity against bacteria (including spores), fungi, viruses, and protozoa.
Pharmacokinetics
Onset
Immediate microbicidal activity upon application.
Excretion
Absorbed iodine is primarily excreted renally.
Half life
The systemic half-life of absorbed iodine is typically short (hours), but can be prolonged in renal impairment. For topical application, localized action is primary.
Absorption
Systemic absorption of iodine from povidone-iodine applied to intact skin is minimal. However, absorption can be significant from large wound areas, burns, or mucous membranes, potentially leading to systemic effects.
Metabolism
Absorbed iodine is metabolized in the body and can affect thyroid function.
Side Effects
Contraindications
- •Known hypersensitivity to iodine or povidone
- •Thyroid disorders (e.g., hyperthyroidism, adenoma, goiter), especially in patients with an overactive thyroid
- •Before and after radioiodine therapy
- •In neonates and infants younger than 6 months (due to risk of hypothyroidism)
Drug Interactions
Lithium
Concurrent use may cause additive thyroid suppression.
Silver sulfadiazine
May reduce the efficacy of both agents.
Hydrogen peroxide, enzymatic preparations
Incompatible, may reduce antiseptic effect.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep away from children.
Overdose
Accidental ingestion or extensive application on large wounds may lead to systemic iodine toxicity, presenting as gastrointestinal upset, metabolic acidosis, renal failure, and thyroid dysfunction (hyper- or hypothyroidism). Management is symptomatic and supportive.
Pregnancy & Lactation
Use with caution during pregnancy and lactation, especially on large areas or for prolonged periods, as absorbed iodine may affect fetal/neonatal thyroid function. Consult a doctor.
