Description
Overview of the medicine
Prasugrel is an antiplatelet medication used to prevent blood clots in patients with acute coronary syndrome (ACS) undergoing percutaneous coronary intervention (PCI). It reduces the risk of cardiovascular events such as heart attack and stroke.
Uses & Indications
Dosage
Adults
Loading dose: 60 mg orally once. Maintenance dose: 10 mg orally once daily. To be taken with aspirin.
Elderly
Generally not recommended for patients ≥75 years due to increased risk of bleeding, unless the benefit outweighs the risk (e.g., diabetes or prior MI). If used, consider 5 mg daily maintenance dose after a 60 mg loading dose.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Data limited for severe renal impairment.
How to Take
Administer orally, with or without food. Swallow the tablet whole. Do not crush or chew.
Mechanism of Action
Prasugrel is a thienopyridine prodrug that requires metabolic activation to its active metabolite. The active metabolite irreversibly binds to the P2Y12 ADP receptors on platelets, thereby preventing ADP-mediated platelet activation and aggregation.

Mainly renal (approximately 68%) and fecal (approximately 27%).
Half life
Plasma half-life of active metabolite is 7.4 hours (range 6-15 hours).
Absorption
Rapidly absorbed and metabolized to its active metabolite. Absolute bioavailability is high.
Metabolism
Rapidly hydrolyzed in the intestine to a thiolactone, then converted by cytochrome P450 enzymes (primarily CYP3A4, CYP2B6, CYP2C9, CYP2C19) to the active metabolite.
Side Effects
Contraindications
- •Active pathological bleeding (e.g., peptic ulcer, intracranial hemorrhage)
- •History of transient ischemic attack (TIA) or stroke
- •Severe hepatic impairment
- •Hypersensitivity to prasugrel or any component of the product.
Drug Interactions
Clopidogrel
Not recommended as concomitant use does not increase efficacy and may increase bleeding risk.
NSAIDs (e.g., Ibuprofen, Naproxen)
Increased risk of bleeding. Use with caution.
Warfarin, Heparin, other Anticoagulants
Increased risk of bleeding. Avoid concomitant use.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include prolonged bleeding. There is no specific antidote for prasugrel. Treatment should be supportive and symptomatic. Platelet transfusions may be considered if bleeding is severe.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is unknown whether prasugrel is excreted in human milk. Breastfeeding is not recommended during treatment.
