Description
Overview of the medicine
Prasugrel is an antiplatelet medication used to prevent blood clots in people with acute coronary syndrome (ACS) who are undergoing a procedure called percutaneous coronary intervention (PCI). It works by preventing platelets from clumping together and forming harmful clots.
Uses & Indications
Dosage
Adults
Following a 60 mg loading dose, the recommended maintenance dose is 10 mg once daily. For patients weighing <60 kg or ≥75 years old, the recommended maintenance dose is 5 mg once daily.
Elderly
For patients ≥75 years of age: A maintenance dose of 5 mg once daily is recommended. Use with caution due to increased bleeding risk.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Prasulet 5 mg tablet should be taken orally, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Prasugrel is a thienopyridine prodrug that is metabolized to an active metabolite. This active metabolite irreversibly binds to the P2Y12 adenosine diphosphate (ADP) receptor on platelets, thereby preventing ADP-mediated platelet activation and aggregation. This action reduces the risk of thrombotic events.

Onset
Within 30 minutes to 1 hour (after loading dose).
Excretion
Excreted primarily via the kidneys (approximately 68%) and feces (approximately 27%) as inactive metabolites.
Half life
The elimination half-life of the active metabolite is approximately 7.4 hours (ranging from 6 to 15 hours).
Absorption
Rapidly absorbed from the gastrointestinal tract and extensively metabolized to its active metabolite. Peak plasma concentration of the active metabolite is reached within approximately 30-60 minutes.
Metabolism
Prasugrel is a prodrug that is rapidly hydrolyzed by carboxylesterase in the intestine to a thiolactone, which is then converted by cytochrome P450 enzymes (primarily CYP3A4, CYP2B6, CYP2C9, CYP2C19) to its active metabolite.
Side Effects
Contraindications
- •Active pathological bleeding (e.g., peptic ulcer, intracranial hemorrhage).
- •History of transient ischemic attack (TIA) or stroke.
- •Severe hepatic impairment.
- •Known hypersensitivity to prasugrel or any component of the product.
Drug Interactions
Warfarin and other anticoagulants
Increased risk of bleeding. Concurrent use is not recommended.
Nonsteroidal Anti-inflammatory Drugs (NSAIDs)
Increased risk of bleeding. Use with caution.
Other Antiplatelet Agents (e.g., Aspirin, Clopidogrel)
Concomitant use with aspirin is indicated; however, use with other antiplatelet agents may increase bleeding risk.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of prasugrel may lead to prolonged bleeding. In case of overdose, seek immediate medical attention. Platelet transfusion may be considered to reverse the antiplatelet effect, but this is not a guaranteed reversal strategy.
Pregnancy & Lactation
Pregnancy Category B. Prasugrel should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether prasugrel is excreted in human milk; caution should be exercised when administered to a nursing mother.
