Description
Overview of the medicine
Prednisolone is a synthetic corticosteroid used to treat a wide variety of inflammatory and autoimmune conditions. It works by decreasing the body's natural defensive response and reducing symptoms such as swelling and allergic-type reactions.
Uses & Indications
Dosage
Adults
Dosage varies widely depending on the condition being treated and patient response. Typically ranges from 5 mg to 60 mg orally once daily or in divided doses. It should be tapered gradually after long-term use.
Elderly
Generally, the lowest effective dose should be used. Increased susceptibility to adverse effects, especially osteoporosis and gastrointestinal issues.
Renal_impairment
No specific dosage adjustment is usually required for renal impairment, but caution is advised in severe cases due to fluid retention.
How to Take
Take orally, preferably with food or milk to minimize gastrointestinal irritation. For once-daily dosing, take in the morning to align with the body's natural cortisol rhythm.
Mechanism of Action
Prednisolone works by binding to glucocorticoid receptors, inhibiting the release of inflammatory mediators (e.g., prostaglandins, leukotrienes), and suppressing immune system activity. It also affects carbohydrate, protein, and fat metabolism.
Pharmacokinetics
Onset
Anti-inflammatory effects typically begin within 1-2 hours; maximal effects may take longer.
Excretion
Excreted mainly in the urine, predominantly as inactive metabolites.
Half life
Plasma half-life is 2-4 hours, but the biological half-life (duration of action) is much longer, typically 18-36 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, achieving peak plasma concentrations within 1-2 hours.
Metabolism
Primarily metabolized in the liver to its active form (prednisone) and inactive metabolites.
Side Effects
Contraindications
- •Systemic fungal infections
- •Known hypersensitivity to prednisolone or any component of the formulation
Drug Interactions
NSAIDs
Increased risk of gastrointestinal bleeding and ulceration.
Warfarin
Can alter anticoagulant effect; monitor INR.
Diuretics
Increased potassium loss.
Live vaccines
Avoid during immunosuppressive doses of corticosteroids.
Insulin/Oral Hypoglycemics
May increase blood glucose levels, requiring dosage adjustment.
Storage
Store at room temperature (20°C-25°C), away from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is rare. Symptoms may include fluid retention, hypertension, and hyperglycemia. Treatment is symptomatic and supportive; no specific antidote. Long-term overdose can lead to Cushingoid features.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Prednisolone is excreted in breast milk; caution advised, and potential risks/benefits should be discussed.
