Dosage
Adults
Initial dose 75 mg twice daily or 50 mg three times daily, gradually increased based on response and tolerability, up to a maximum of 300 mg twice daily or 200 mg three times daily (total 600 mg/day).
Elderly
May require lower doses due to potential for decreased renal function. Dose titration should be gradual.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance. For CrCl 30-60 mL/min, 25-50 mg twice daily or 50-100 mg once daily. For CrCl <15 mL/min, 25-50 mg once daily.
How to Take
