Description
Overview of the medicine
Pregabalin is an anticonvulsant and neuropathic pain agent. It is used to treat neuropathic pain, fibromyalgia, partial-onset seizures, and generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose 75 mg twice daily or 50 mg three times daily (150 mg/day). May be increased to a maximum of 300 mg twice daily or 600 mg daily.
Elderly
Dose adjustment may be necessary due to age-related decrease in renal function. Start with a lower dose and titrate slowly.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl 30-60 mL/min: 75 mg/day in 2-3 divided doses. For CrCl 15-30 mL/min: 25-50 mg/day once daily or every other day.
How to Take
Oral administration, with or without food. Swallow the capsule whole with water.
Mechanism of Action
Pregabalin binds to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system, reducing the release of excitatory neurotransmitters (e.g., glutamate, norepinephrine, substance P). This leads to analgesic, anxiolytic, and anticonvulsant effects.
Pharmacokinetics
Onset
Analgesic effects may take several days to weeks to become apparent. Onset of action for GAD can be within 1 week.
Excretion
Excreted primarily unchanged in the urine (approximately 98%).
Half life
Approximately 6.3 hours.
Absorption
Rapidly and well absorbed after oral administration; bioavailability is >90% and is not affected by food.
Metabolism
Negligible hepatic metabolism in humans (<2% of a dose).
