Premesis 8 mg Injection is used to prevent and treat nausea and vomiting caused by cancer chemotherapy, radiation therapy, or surgery.
Uses & Indications
Dosage
Adults
Chemotherapy/Radiotherapy induced nausea and vomiting: 8-24 mg (single or divided doses depending on emetogenicity). Post-operative nausea and vomiting: 4 mg as a single IV or IM dose.
Elderly
No specific dose adjustment is usually required, but hepatic function should be monitored.
Renal_impairment
No dose adjustment necessary.

Mechanism of Action
Ondansetron works by blocking the action of serotonin (a natural substance) in the brain and gut that can cause nausea and vomiting. It selectively antagonizes 5-HT3 receptors located in the chemoreceptor trigger zone (CTZ) and peripherally on vagal nerve terminals.

Side Effects
Contraindications
- •Hypersensitivity to ondansetron or any component of the formulation.
- •Concomitant use with apomorphine due to the risk of profound hypotension and loss of consciousness.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol.
Apomorphine
Severe hypotension and loss of consciousness.
Phenytoin, Carbamazepine, Rifampicin
May decrease ondansetron plasma concentrations.
QT-prolonging drugs (e.g., Amiodarone, Erythromycin)
Increased risk of QT prolongation and Torsade de Pointes.
Storage
Store below 25°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include transient visual disturbances, severe constipation, hypotension, and vasovagal episodes with transient AV block. ECG abnormalities, including QT prolongation, have been reported. Management is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Generally avoided in the first trimester. Use with caution during pregnancy only if clearly needed and the potential benefits outweigh the risks. Ondansetron is excreted in breast milk; use with caution during lactation.
