Overview of the medicine
Prenep-50 mg Capsule is primarily used to treat neuropathic pain, generalized anxiety disorder, and fibromyalgia. It works by calming overactive nerves in the brain and body.
Uses & Indications
Dosage
Adults
Initial dose: 50 mg three times daily (150 mg/day). May be increased to 100 mg three times daily (300 mg/day) within one week. Maximum dose: 600 mg/day.
Elderly
Starting dose may be lower (e.g., 25 mg two to three times daily) due to potential age-related decline in renal function.
Renal_impairment
Dosage adjustment is necessary. For CrCl 30-60 mL/min: 75 mg/day in 2-3 divided doses. For CrCl 15-30 mL/min: 25-50 mg/day in 1-2 divided doses. For CrCl <15 mL/min: 25 mg once daily or 25 mg every other day.
How to Take
Take orally with or without food. Swallow the capsule whole with water. Do not crush, chew, or open the capsule.
Mechanism of Action
Prenep binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system, reducing the release of several neurotransmitters including glutamate, norepinephrine, and substance P. This action is believed to mediate its analgesic and anxiolytic effects.
Pharmacokinetics
Onset
Within 1 week for neuropathic pain relief, 2-4 weeks for anxiety.
Excretion
Excreted almost entirely unchanged in the urine; renal clearance is directly proportional to creatinine clearance.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations typically within 1.5 hours. Bioavailability is ≥90% and is not affected by food.
Metabolism
Negligible metabolism in humans; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to Prenep or any components of the formulation.
Drug Interactions
Antihypertensive agents
May potentiate the hypotensive effect when co-administered.
CNS Depressants (e.g., Opioids, Benzodiazepines, Alcohol)
May enhance the central nervous system (CNS) depressant effects, including somnolence and dizziness. Concomitant use should be approached with caution.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and restlessness. Management should involve supportive care and symptomatic treatment. Hemodialysis can be effective in removing Prenep from the systemic circulation.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Prenep is excreted in human milk; caution should be exercised when administered to a nursing mother.
