Description
Overview of the medicine
Prenep 75 mg Capsule contains Pregabalin, an antiepileptic drug used to treat neuropathic pain, fibromyalgia, and as an adjunct therapy for partial-onset seizures. It can also be used for generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose 75 mg twice daily (150 mg/day) or 50 mg three times daily (150 mg/day), adjusted based on response up to 300-600 mg/day in divided doses.
Elderly
Dose adjustments may be necessary due to potential age-related decrease in renal function. Start at lower end of dosage range.
Renal_impairment
Dose reduction required for creatinine clearance < 60 mL/min. Specific dosing based on CrCl.
How to Take
Take orally with or without food. Swallow the capsule whole with water. Do not crush or chew.
Mechanism of Action
Pregabalin binds with high affinity to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system. This binding modulates calcium influx, thereby reducing the release of several excitatory neurotransmitters such as glutamate, noradrenaline, and substance P, which are involved in nociception and seizure generation.

Onset
Analgesic effects may be noticeable within one week; anxiolytic effects may be quicker.
Excretion
Excreted predominantly unchanged in the urine; renal elimination is the major route.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations typically occur within 1.5 hours.
Metabolism
Negligible metabolism in humans; less than 0.1% of the dose is metabolized.
Side Effects
Contraindications

Drug Interactions
ACE inhibitors
May lead to increased risk of angioedema when co-administered with pregabalin.
CNS Depressants (e.g., alcohol, benzodiazepines, opioids)
May enhance the central nervous system (CNS) depressant effects of pregabalin, leading to increased drowsiness and sedation.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and restlessness. Treatment is supportive and symptomatic; hemodialysis may be effective for severe overdose.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted in human milk; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
