Description
Overview of the medicine
Valsartan is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension), heart failure, and to improve survival after a heart attack.
Uses & Indications
Dosage
Adults
Hypertension: Initial 80 mg or 160 mg once daily. Max 320 mg/day. Heart Failure: Initial 40 mg twice daily. Max 320 mg/day. Post-MI: Initial 20 mg twice daily, titrated up to 160 mg twice daily.
Elderly
No dose adjustment usually needed based on age, but monitor renal function.
Renal_impairment
No dose adjustment for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Take orally, with or without food. Administer consistently at about the same time each day.
Mechanism of Action
Valsartan selectively blocks the angiotensin II type 1 (AT1) receptor, preventing angiotensin II from binding to these receptors. This leads to vasodilation, decreased aldosterone secretion, reduced sympathetic activity, and decreased renal reabsorption of sodium and water, thus lowering blood pressure.

Onset
Antihypertensive effect within 2 hours, full effect within 2-4 weeks.
Excretion
Primarily excreted in feces (83%) and urine (13%) as unchanged drug.
Half life
Approximately 6 hours.
Absorption
Bioavailability approx 23%. Peak plasma concentration 2-4 hours.
Metabolism
Minimally metabolized (less than 20%) by CYP450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to valsartan or any component
- Pregnancy (second and third trimesters)
- Concomitant use with aliskiren in diabetic patients or those with renal impairment
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Increased risk of renal impairment, hypotension, and hyperkalemia (especially in diabetics).
NSAIDs (including COX-2 inhibitors)
Decreased antihypertensive effect, increased risk of renal impairment.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia.

Store below 30°C. Protect from moisture and light. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension and dizziness. Management involves supportive care, primarily intravenous saline. Valsartan is not removed by hemodialysis.
Pregnancy & Lactation
Contraindicated in the second and third trimesters of pregnancy due to fetal toxicity. Not recommended during breastfeeding due to potential risk to the infant.
