Description
Overview of the medicine
Presart 80 mg Tablet contains Valsartan, an angiotensin II receptor blocker (ARB), used to treat high blood pressure, heart failure, and to improve survival after a heart attack.
Uses & Indications
Dosage
Adults
Hypertension: 80 mg once daily, adjusted up to 320 mg/day. Heart failure: Initial 40 mg twice daily, titrated up to 160 mg twice daily. Post-MI: Initial 20 mg twice daily, titrated up to 160 mg twice daily.
Elderly
No dosage adjustment generally needed for elderly patients with normal renal function.
Renal_impairment
No initial dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe renal impairment (CrCl <10 mL/min) or dialysis.
How to Take
Taken orally, with or without food, once or twice daily as prescribed by a doctor.
Mechanism of Action
Valsartan selectively blocks the binding of angiotensin II to the AT1 receptor, inhibiting the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation, decreased blood pressure, and reduced fluid retention.
Pharmacokinetics
Onset
Antihypertensive effect within 2 hours, maximal effect within 4-6 hours.
Excretion
Primarily excreted in feces (approximately 83%) via bile, and to a lesser extent in urine (approximately 13%) as unchanged drug.
Half life
Approximately 6-9 hours.
Absorption
Rapidly absorbed after oral administration. Absolute bioavailability is approximately 23%. Food decreases absorption (AUC by 40%, Cmax by 50%).
Metabolism
Minimally metabolized; only about 20% of the absorbed dose is converted to an inactive metabolite (valeryl-4-hydroxy valsartan).
Side Effects
Contraindications
- Hypersensitivity to valsartan or any component of the formulation.
- Concurrent use with aliskiren in patients with diabetes or renal impairment.
- Pregnancy (especially second and third trimesters).
Drug Interactions
NSAIDs
May reduce antihypertensive effect and increase risk of renal impairment.
Lithium
Increased serum lithium concentrations and toxicity.
Diuretics
Increased risk of hypotension.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include profound hypotension and tachycardia; bradycardia could also occur. Management is supportive, including intravenous fluid administration and observation. Hemodialysis is unlikely to remove valsartan effectively.
Pregnancy & Lactation
Pregnancy: Contraindicated, especially in 2nd and 3rd trimesters due to fetal toxicity. Lactation: Not recommended, as it is unknown if valsartan is excreted in human milk. A decision should be made whether to discontinue nursing or discontinue the drug.
