Description
Overview of the medicine
Presart-HZ 80 mg Tablet is a combination medicine containing Valsartan and Hydrochlorothiazide, used to treat essential hypertension (high blood pressure) when monotherapy is not sufficient.
Uses & Indications
Dosage
Adults
The usual starting dose is one Presart-HZ 80 mg tablet once daily. The dose may be increased after 3-4 weeks if blood pressure control is not adequate, up to a maximum of 320 mg valsartan / 25 mg hydrochlorothiazide daily.
Elderly
No specific dose adjustment is required in elderly patients, but renal function should be monitored.
Renal_impairment
Not recommended for patients with severe renal impairment (creatinine clearance <30 mL/min). No dose adjustment needed for mild to moderate impairment.
How to Take
Take the tablet orally, with or without food, usually once daily at approximately the same time each day.
Mechanism of Action
Valsartan, an ARB, selectively blocks the binding of angiotensin II to the AT1 receptor, inhibiting vasoconstriction and aldosterone-secreting effects. Hydrochlorothiazide, a thiazide diuretic, increases the excretion of sodium and chloride, and thus water, from the body by inhibiting reabsorption in the distal convoluted tubules of the nephron.
Pharmacokinetics
Onset
Within 2 hours (blood pressure reduction).
Excretion
Valsartan: Primarily via feces (70%) and urine (30%). Hydrochlorothiazide: Primarily via urine (at least 95% unchanged).
Half life
Valsartan: 6-9 hours. Hydrochlorothiazide: 5.6-14.8 hours.
Absorption
Valsartan: Bioavailability ~25%. Hydrochlorothiazide: Bioavailability ~65-75%. Peak plasma concentrations within 2-4 hours for both components.
Metabolism
Valsartan: Minimally metabolized (~20%) to an inactive metabolite. Hydrochlorothiazide: Not metabolized.
Side Effects
Contraindications
- Hypersensitivity to valsartan, hydrochlorothiazide, or any component of the formulation.
- Anuria.
- Severe renal impairment (creatinine clearance <30 mL/min).
- Severe hepatic impairment, biliary cirrhosis, and cholestasis.
- Pregnancy (2nd and 3rd trimesters).
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR <60 mL/min/1.73 m²).
Drug Interactions
NSAIDs
Reduced antihypertensive effect and increased risk of renal impairment.
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Contraindicated in diabetic or renally impaired patients (GFR <60 mL/min/1.73 m²).
Other antihypertensives
Additive hypotensive effects.
Cholestyramine/Colestipol
Reduced absorption of hydrochlorothiazide.
Alcohol, barbiturates, narcotics
Potentiation of orthostatic hypotension.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia.

Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.

Symptoms may include marked hypotension, tachycardia, bradycardia, electrolyte disturbances, and dehydration. Treatment is symptomatic and supportive, including correction of electrolyte imbalances.
Pregnancy & Lactation
Contraindicated in the 2nd and 3rd trimesters of pregnancy due to fetal toxicity. Not recommended during lactation as hydrochlorothiazide is excreted in breast milk.
