Description
Overview of the medicine
Primace 10 mg capsule contains Omeprazole, a proton pump inhibitor used to reduce stomach acid production. It is commonly prescribed for conditions like heartburn, acid reflux, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD maintenance: 10 mg once daily. For H. pylori eradication: 10 mg twice daily in combination with antibiotics. Other indications usually require 20-40mg.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take the capsule whole with water at least 30 minutes before a meal, preferably in the morning. Do not chew or crush the capsule.
Mechanism of Action
Omeprazole inhibits the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This irreversibly blocks the final step in acid production, leading to a sustained decrease in gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Mainly excreted in urine (about 80%) as metabolites, with a smaller portion excreted in feces.
Half life
0.5-1 hour (plasma half-life), but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed from the gastrointestinal tract after oral administration. Bioavailability is approximately 30-40% at low doses, increasing to about 65% with repeated doses.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Omeprazole or any component of the formulation
- Concomitant use with Nelfinavir
Drug Interactions
Diazepam
Reduced clearance, increased half-life
Warfarin
Increased INR/prothrombin time
Tacrolimus
Increased tacrolimus levels
Clopidogrel
Reduced antiplatelet effect
Methotrexate
Increased plasma levels of methotrexate
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and include drowsiness, headache, tachycardia, nausea, vomiting, and abdominal pain. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C (US FDA). Use only if the potential benefit outweighs the potential risk to the fetus. Small amounts of omeprazole may be excreted in breast milk; caution should be exercised when administered to a nursing mother.

Lab Monitoring
- Monitor for magnesium levels with prolonged use (especially >1 year) or with concomitant diuretics.
- Monitor INR for patients on warfarin.
Doctor Notes
- Advise patients on appropriate lifestyle modifications alongside medication.
- Consider lowest effective dose for shortest duration possible, especially for long-term use.
- Regularly assess the need for continued PPI therapy.
Patient Guidelines
- Take the capsule whole; do not crush or chew.
- Take at least 30 minutes before a meal, usually in the morning.
