Description
Overview of the medicine
Cefixime is a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections including those of the respiratory tract, urinary tract, and otitis media. It works by inhibiting bacterial cell wall synthesis.
Uses & Indications
Dosage
Adults
Typically not used as suspension for adults; usually 400 mg once daily or 200 mg every 12 hours as tablets. If suspension used, adjust dose accordingly.
Elderly
No dosage adjustment required unless severe renal impairment (CrCl < 60 mL/min).
Children
6 months to 10 years or under 50 kg: 8 mg/kg/day, given as a single dose or in two divided doses. For 125 mg/5 ml suspension: 5 ml (125 mg) per 12.5 kg body weight.
Renal_impairment
CrCl 21-60 mL/min: 75% of standard dose. CrCl < 20 mL/min: 50% of standard dose.
How to Take
Take orally. Can be taken with or without food. Shake the bottle well before each use to ensure proper mixing. Use the provided measuring device for accurate dosing.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Clinical effects typically observed within 1-2 hours.
Excretion
Primarily excreted unchanged in urine (about 50%) and bile (about 10%) within 24 hours.
Half life
Approximately 3-4 hours, which allows for once or twice daily dosing.
Absorption
Well absorbed orally, with a bioavailability of approximately 40-50%. Peak plasma concentrations are achieved 2-6 hours after administration.
Metabolism
Minimally metabolized; 10% to 15% is metabolized to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics.
Drug Interactions
Warfarin
May increase prothrombin time (PT) and international normalized ratio (INR) with or without bleeding, requiring anticoagulant dose adjustment.
Nifedipine
Concurrent use may increase cefixime Cmax and AUC.
Carbamazepine
Increased carbamazepine levels have been reported when co-administered with cefixime.
Oral anticoagulants
Enhanced anticoagulant effect, careful monitoring of coagulation parameters is recommended.
Storage
Store the dry powder below 30°C, protected from light and moisture. After reconstitution, store the suspension in the refrigerator (2-8°C) and do not freeze. Discard any unused portion after the recommended period.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote for cefixime. Treatment should be symptomatic and supportive. Hemodialysis and peritoneal dialysis are not effective in removing cefixime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in small amounts in human breast milk; use with caution in breastfeeding mothers.

Lab Monitoring
- Renal function tests (blood urea nitrogen, creatinine) if prolonged therapy or pre-existing renal impairment.
- Liver function tests (alanine transaminase, aspartate transaminase) in patients with hepatic dysfunction or during prolonged treatment.
- Complete blood count (CBC) if prolonged treatment, to monitor for hematological abnormalities.
Doctor Notes
- Ensure parents/caregivers are thoroughly instructed on proper reconstitution, measurement, and administration of the suspension.
- Be vigilant for signs of hypersensitivity reactions, especially in patients with a history of penicillin allergy, as cross-reactivity can occur.
- Emphasize the importance of completing the full prescribed course to prevent treatment failure and reduce resistance.
Patient Guidelines
- Complete the entire course of medication as prescribed by your doctor, even if symptoms improve.
- Shake the suspension vigorously before each use to ensure the medication is evenly mixed.
- Use the provided measuring spoon or cup to accurately measure each dose.
- Store the reconstituted suspension in the refrigerator and discard any unused portion after the recommended period (usually 7-14 days).
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Cefixime may cause dizziness in some individuals. Patients should be cautioned about driving or operating machinery until they know how the medication affects them.
Lifestyle Advice
- Maintain good hygiene to prevent the spread of infection and reinfection.
- Drink plenty of fluids to stay hydrated, especially if experiencing diarrhea.
- Avoid self-medication with antibiotics to prevent antibiotic resistance.
