Description
Overview of the medicine
Prioxim 200 mg Tablet contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections affecting the respiratory tract, urinary tract, and ears.
Uses & Indications
Dosage
Adults
Typically 200 mg once daily or 100 mg twice daily for 7-14 days depending on infection severity. For uncomplicated gonorrhea, a single oral dose of 400 mg.
Elderly
No dose adjustment generally required unless renal function is severely impaired.
Renal_impairment
For creatinine clearance < 30 mL/min, the dose should be reduced to 100 mg once daily.
How to Take
Prioxim 200 mg Tablet can be taken with or without food. To minimize potential gastrointestinal upset, it may be taken with food. It is crucial to complete the entire prescribed course.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, thereby preventing the cross-linking of peptidoglycan units, which leads to cell lysis and bacterial cell death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after administration.
Excretion
Mainly excreted by the kidneys (approximately 50% unchanged in urine) and some biliary excretion.
Half life
About 3-4 hours.
Absorption
Approximately 30-50% absorbed from the gastrointestinal tract; absorption is not significantly affected by food, but may be slightly increased.
Metabolism
Not extensively metabolized in the body; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics
- Known hypersensitivity to penicillin (due to potential cross-reactivity)
Drug Interactions
Warfarin
May increase prothrombin time (PT) and international normalized ratio (INR), requiring close monitoring.
Carbamazepine
Concomitant use may increase carbamazepine plasma levels.
Aminoglycosides
Increased risk of nephrotoxicity with concurrent use.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; advise using alternative birth control.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
No specific antidote exists. Management should be symptomatic and supportive. Gastric lavage may be considered if ingestion is recent. Cefixime is not effectively removed by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Animal studies show no risk, but human data is limited. Use only if clearly needed. Excreted in low concentrations in breast milk; generally considered safe, but monitor infant for potential adverse effects like diarrhea or candidiasis.
