Description
Overview of the medicine
Prochlorperazine is a phenothiazine derivative used primarily as an antiemetic for severe nausea and vomiting. It is also used for the short-term management of severe anxiety and psychotic disorders.
Uses & Indications
Dosage
Adults
For nausea/vomiting: 5-10 mg 3-4 times daily. For anxiety: 5 mg 3-4 times daily. For psychosis: 10-25 mg 2-3 times daily, adjusted as needed.
Elderly
Lower doses may be required due to increased sensitivity and slower metabolism. Start with 2.5-5 mg once or twice daily.
Renal_impairment
No specific dose adjustment for mild to moderate impairment; use with caution in severe impairment.
How to Take
Oral. Can be taken with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
It exerts its antiemetic effect by blocking dopamine D2 receptors in the chemoreceptor trigger zone (CTZ) in the brain. Its antipsychotic action involves blockage of postsynaptic mesolimbic dopaminergic D1 and D2 receptors in the brain.
Pharmacokinetics
Onset
Antiemetic effect: 30-40 minutes (oral). Antipsychotic effect: days to weeks.
Excretion
Excreted via urine and feces as metabolites.
Half life
Variable, typically 6-8 hours for antiemetic effects; longer for antipsychotic effects (e.g., 20-40 hours for elimination).
Absorption
Well absorbed orally, but undergoes significant first-pass metabolism. Peak plasma concentrations reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver (CYP450 enzymes).
Side Effects
Contraindications
- Hypersensitivity to prochlorperazine or other phenothiazines
- Comatose states
- Severe central nervous system (CNS) depression
- Bone marrow depression
- Blood dyscrasias
- Children under 2 years or weighing less than 9 kg
Drug Interactions
Anticholinergics
Potentiated anticholinergic effects, increased risk of paralytic ileus and heatstroke.
Antihypertensives
Increased risk of orthostatic hypotension.
Dopamine agonists (e.g., levodopa)
Reduced efficacy of dopamine agonists.
CNS depressants (alcohol, sedatives, opioids)
Enhanced sedative effects and respiratory depression.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe drowsiness, low blood pressure, muscle rigidity, tremors, unresponsiveness, and extrapyramidal symptoms. Treatment is supportive; gastric lavage, activated charcoal, IV fluids for hypotension, antiparkinsonian drugs for extrapyramidal symptoms.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Excreted in breast milk; avoid breastfeeding or discontinue drug due to potential for serious adverse effects in the infant.

- Liver function tests (LFTs) (periodically)
- Blood pressure monitoring (for orthostatic hypotension)
Doctor Notes
- Counsel patients on potential for drowsiness and extrapyramidal symptoms (EPS).
- Monitor for signs of Neuroleptic Malignant Syndrome (NMS), especially with fever, muscle rigidity, and altered mental status.
- Consider the lowest effective dose for the shortest duration, particularly in elderly patients and those at risk for tardive dyskinesia.
- Avoid abrupt discontinuation to prevent withdrawal-like symptoms.
Patient Guidelines
- Do not drive or operate heavy machinery until you know how this medicine affects you.
- Avoid alcohol and other CNS depressants while taking this medicine.
- Report any unusual muscle movements, fever, or changes in mental status to your doctor immediately.
- Do not stop taking this medicine abruptly without consulting your doctor, as withdrawal symptoms may occur.
