Overview of the medicine
Proflam SR (Diclofenac Sodium Sustained Release) is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and stiffness caused by various conditions like arthritis, muscle aches, and injuries. The sustained-release formulation provides prolonged therapeutic effect.
Uses & Indications
Dosage
Adults
Typically 100 mg once daily, preferably with or after food. Maximum recommended dose is generally 100-150 mg/day depending on patient response and tolerability.
Elderly
Dosage should be initiated at the lower end of the dosing range and monitored for adverse effects due to increased risk in elderly patients, especially regarding renal function.
Renal_impairment
Contraindicated in severe renal impairment. In mild to moderate impairment, use with caution and consider dose reduction; monitor renal function.
How to Take
Take the tablet whole with a glass of water, preferably with or immediately after food to minimize gastrointestinal upset. Do not crush, chew, or break the tablet.
Mechanism of Action
Diclofenac inhibits prostaglandin synthesis by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes. Prostaglandins are mediators of inflammation, pain, and fever, so by inhibiting their synthesis, diclofenac reduces these symptoms.

Onset
Slower onset of action due to sustained release, but provides prolonged therapeutic effect.
Excretion
Excreted mainly by the kidneys (approximately 60%) and to a lesser extent by the bile/feces (approximately 30%).
Half life
Approximately 1-2 hours for the parent drug, but the sustained-release mechanism ensures a longer duration of action.
Absorption
Well absorbed after oral administration; sustained-release formulation delays peak plasma levels compared to immediate-release forms.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzyme CYP2C9, to inactive metabolites.

Drug Interactions
SSRIs

Lithium, Methotrexate, Digoxin
Increased plasma levels of these drugs.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Other NSAIDs or corticosteroids
Increased risk of gastrointestinal adverse effects.
Antiplatelets (e.g., Aspirin, Clopidogrel)
Increased risk of gastrointestinal bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management is supportive and symptomatic. Gastric lavage and/or activated charcoal may be considered within one hour of ingestion.
Pregnancy & Lactation
Pregnancy Category C (Category D in the third trimester). Avoid use in late pregnancy due to potential fetal harm and prolonged labor. Diclofenac is excreted in breast milk in small amounts; consult a doctor before use during lactation.
