Overview of the medicine
Proflox 250 mg Tablet contains Ciprofloxacin, a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections, including urinary tract infections, respiratory tract infections, skin and soft tissue infections, bone and joint infections, and certain sexually transmitted infections.
Uses & Indications
Dosage
Adults
Typical dosage is 250-750 mg orally twice daily, depending on the infection type and severity. For uncomplicated UTIs, 250 mg twice daily for 3 days.
Elderly
Dosage adjustment may be necessary based on renal function.
Renal_impairment
Dosage reduction is required for patients with creatinine clearance less than 50 mL/min. For CrCl 30-50 mL/min, administer 250-500 mg every 12 hours. For CrCl < 30 mL/min, administer 250-500 mg every 18-24 hours.
How to Take
Take orally with a glass of water, with or without food. Do not take with dairy products or calcium-fortified juices alone, as this may decrease absorption. Separate from antacids, iron, or zinc supplements by at least 2 hours.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and ultimately bacterial cell death.
Pharmacokinetics
Onset
Varies depending on infection type, generally within 1-2 hours for therapeutic levels.
Excretion
Primarily excreted renally (50-70% as unchanged drug) and to a lesser extent via the feces (15% as unchanged drug and metabolites).
Half life
Approximately 3-5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, bioavailability is about 70-80%.
Metabolism
Undergoes limited hepatic metabolism (10-20%) to several less active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to ciprofloxacin or other fluoroquinolones
- •Concomitant use with Tizanidine
Drug Interactions
Warfarin
Enhanced anticoagulant effects, requiring INR monitoring.
Tizanidine
Increased risk of hypotension and sedation due to increased Tizanidine plasma concentrations.
Theophylline
Increased plasma levels of Theophylline, potentially leading to toxicity.
Corticosteroids
Increased risk of tendon rupture.
Antacids (containing aluminum, magnesium, calcium), Iron, Zinc supplements
Decreased absorption of ciprofloxacin. Administer at least 2 hours before or 6 hours after.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of acute overdose, reversible renal toxicity has been reported. Gastric emptying should be induced, or activated charcoal administered. Patients should be carefully observed and given supportive treatment, including maintaining adequate hydration. Hemodialysis or peritoneal dialysis remove only small amounts of ciprofloxacin.
Pregnancy & Lactation
Pregnancy Category C. Ciprofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Ciprofloxacin is excreted in human milk and may cause serious adverse reactions in nursing infants. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
