Description
Overview of the medicine
Propin 40 mg Capsule contains Omeprazole, a proton pump inhibitor used to reduce the amount of acid produced in the stomach. It is prescribed for conditions like gastroesophageal reflux disease (GERD), peptic ulcer disease, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Duodenal Ulcer: 20 mg once daily for 2-4 weeks. Gastric Ulcer: 20 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial dose 60 mg once daily, adjusted as needed.
Elderly
No specific dosage adjustment is generally required, but caution is advised in very elderly patients due to potential decreased metabolism.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take one capsule orally, usually in the morning before a meal. Swallow the capsule whole with water; do not chew or crush it.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase proton pump in gastric parietal cells. This action blocks the final step of acid production, leading to a profound and long-lasting reduction in gastric acid secretion.
Pharmacokinetics
Onset
Acid suppression begins within 30-60 minutes.
Excretion
Approximately 80% of metabolites are excreted in the urine, and the remainder in the feces.
Half life
Plasma half-life is short, about 0.5 to 1 hour, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 0.5 to 3.5 hours. Bioavailability is approximately 30-40% but increases with repeated dosing.
Metabolism
Extensively metabolized in the liver, primarily via the cytochrome P450 enzyme system, specifically CYP2C19 and CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
May increase serum digoxin levels.
Warfarin
Potentiates the anticoagulant effect of warfarin, requiring INR monitoring.
Tacrolimus
May increase tacrolimus levels.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting its conversion to active metabolite.
Methotrexate
May increase methotrexate levels, especially with high-dose methotrexate.
Storage
Store below 30°C in a cool, dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, confusion, blurred vision, tachycardia, nausea, sweating, flushing, headache, and dry mouth. Management is symptomatic and supportive, as there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Omeprazole is excreted in breast milk; therefore, caution should be exercised when administered to a nursing woman.

Lab Monitoring
- No routine monitoring is required. However, long-term use (more than 3 months) may necessitate monitoring of serum magnesium levels. Vitamin B12 levels may also be monitored in long-term therapy.
Doctor Notes
- Assess for symptoms of underlying malignancy before initiating treatment, especially in patients with alarm symptoms (e.g., unexplained weight loss, dysphagia).
- Use the lowest effective dose for the shortest duration necessary.
- Re-evaluate the need for continued PPI therapy periodically, particularly for long-term use.
