Description
Overview of the medicine
Prostol 1 mg Capsule contains Misoprostol, a synthetic prostaglandin E1 analog. While Misoprostol is known for preventing NSAID-induced gastric ulcers, the 1 mg dose is significantly higher than typical gastroprotective doses (usually 100-200 mcg) and suggests its primary use in obstetrics for specific indications like cervical ripening, labor induction, or management of postpartum hemorrhage, under strict medical supervision. This specific 1 mg capsule formulation might be for specialized use or compounded preparations.
Uses & Indications
Dosage
Adults
Dosage is highly variable and depends on the specific obstetric indication. For labor induction/cervical ripening, typically lower doses (e.g., 25-50 mcg) are given repeatedly. A 1 mg dose is exceptionally high and would only be considered for very specific, acute, and medically supervised situations, such as severe postpartum hemorrhage, or according to specialized protocols. It is NOT for routine GI prophylaxis.
Elderly
No specific dosing recommendations for the elderly for obstetric indications, as this is usually not relevant. For other potential uses, Misoprostol dosage may need adjustment due to reduced renal function in the elderly. However, 1 mg is a very high dose.
Renal_impairment
In patients with renal impairment, Misoprostol acid exposure may be increased. Dosage reduction may be necessary for general Misoprostol use, but for 1 mg in acute obstetric settings, careful risk-benefit assessment by a specialist is required.
How to Take
Prostol 1 mg Capsule is for oral administration. It should be taken exactly as prescribed by the doctor, often with food to minimize gastrointestinal upset. Due to the high dose, this preparation is expected to be administered only in a clinical setting by trained medical professionals.
Mechanism of Action
Misoprostol is a synthetic prostaglandin E1 analog. In the gastrointestinal tract, it reduces gastric acid secretion by directly inhibiting the parietal cell proton pump and enhances mucosal defense by increasing bicarbonate and mucus production, and improving mucosal blood flow. In the uterus, it binds to specific prostaglandin receptors on myometrial cells, causing strong uterine contractions and cervical softening and dilation, which can lead to expulsion of uterine contents.
Pharmacokinetics
Onset
GI effects can begin within 30 minutes. Uterine contractions typically begin within 10-20 minutes after vaginal administration, and 30-60 minutes after oral administration.
Excretion
Approximately 80% of the active metabolite is excreted in the urine, and the remainder in feces.
Half life
The plasma elimination half-life of misoprostol acid is short, approximately 20-40 minutes.
Absorption
Rapidly absorbed from the GI tract and extensively de-esterified to misoprostol acid (the active metabolite). Peak plasma concentrations of misoprostol acid occur in about 12 minutes (oral administration).
Metabolism
Extensive hepatic metabolism via fatty acid oxidation system, resulting in inactivation.
Side Effects
Contraindications
- •Known hypersensitivity to misoprostol, other prostaglandins, or any component of the capsule.
- •Pregnancy, if used for the prevention of NSAID-induced ulcers (as it is an abortifacient and teratogenic).
- •For obstetric indications, contraindications relate to fetal distress, previous uterine surgery (e.g., C-section with uncertain uterine integrity), or placenta previa.
Drug Interactions
NSAIDs
While misoprostol is used to prevent NSAID-induced ulcers, excessive concomitant use of NSAIDs may still carry gastrointestinal risks.
Oxytocin
Concurrent use with oxytocin for labor induction or augmentation may increase the risk of uterine hyperstimulation and uterine rupture, requiring careful monitoring and dose adjustment.
Antacids (especially magnesium-containing)
May exacerbate diarrhea caused by misoprostol.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include sedation, tremor, convulsions, dyspnea, abdominal pain, diarrhea, fever, palpitations, and hypotension. Treatment is supportive and symptomatic, as there is no specific antidote. Dialysis is not expected to be beneficial due to rapid metabolism.
Pregnancy & Lactation
Misoprostol is a known abortifacient and teratogen. It is absolutely contraindicated in pregnant women when used for ulcer prevention. For obstetric indications, it is used for medical abortion, cervical ripening, and labor induction under strict medical supervision and risk-benefit assessment. Misoprostol acid is excreted in breast milk. Caution should be exercised when administered to a nursing woman, and alternative feeding methods may be considered.
