Description
Overview of the medicine
Prostol (Misoprostol) is a synthetic prostaglandin E1 analogue that helps protect the stomach lining by reducing acid secretion and enhancing mucosal defense. It also has uterotonic effects, causing uterine contractions and cervical ripening, and is used for various obstetric and gynecological indications.
Uses & Indications
Dosage
Adults
For specific indications requiring high doses, such as certain obstetric procedures or severe refractory gastric conditions, under strict medical supervision. Typical Misoprostol doses for ulcer prevention are 200 mcg (0.2 mg) four times daily. For obstetric uses, doses range from 25 mcg to 800 mcg, depending on the indication and route of administration. A 2mg capsule is a very high dose and should only be used under expert medical guidance for highly specific and severe conditions.
Elderly
No specific dose adjustment for elderly patients for ulcer prevention; use with caution due to potential increased sensitivity. For other uses, follow specific medical guidance and monitor closely for adverse effects.
Renal_impairment
Reduce dose in patients with severe renal impairment (CrCl <30 mL/min). Consider 50% of the usual dose and monitor for adverse effects.
How to Take
Swallow the capsule whole with water, preferably after food to minimize gastrointestinal side effects. For obstetric uses, administration route and method (oral, vaginal, sublingual) depend on the specific indication and should be strictly guided by a healthcare professional.
Mechanism of Action
Misoprostol inhibits gastric acid secretion, stimulates mucosal bicarbonate and mucus production, and enhances mucosal blood flow, thereby protecting the gastric mucosa. It binds to prostaglandin receptors on myometrial cells, leading to myometrial contractions and cervical ripening.
Pharmacokinetics
Onset
Gastric effects within 30 minutes. Uterine effects within 10-20 minutes when administered vaginally or orally.
Excretion
Primarily via urine (80%) and feces (15%) within 24 hours.
Half life
Misoprostol acid half-life is 20-40 minutes.
Absorption
Rapidly absorbed orally, quickly de-esterified to misoprostol acid, the active metabolite. Food may decrease absorption rate but not extent.
Metabolism
Extensively metabolized in the liver to the active misoprostol acid.
Side Effects
Contraindications
- •Known hypersensitivity to misoprostol or other prostaglandins
- •Pregnancy (unless used for labor induction or abortion under strict medical supervision)
- •Ectopic pregnancy
- •Active gastrointestinal bleeding
- •History of uterine surgery or rupture (for obstetric use)
Drug Interactions
Oxytocin
Increased risk of uterine hyperstimulation when co-administered for labor induction. Careful monitoring is required.
Other NSAIDs
May increase risk of gastrointestinal side effects.
Antacids containing magnesium
May worsen diarrhea.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include sedation, tremor, convulsions, dyspnea, abdominal pain, diarrhea, fever, palpitations, and bradycardia. Management is supportive and symptomatic; there is no specific antidote. A 2mg dose as a single capsule is significantly higher than typical therapeutic doses for Misoprostol and could lead to severe adverse effects.
Pregnancy & Lactation
Contraindicated in pregnancy unless used for medical abortion or labor induction under strict medical supervision due to its abortifacient and teratogenic risks. Misoprostol and its active metabolite are excreted in breast milk; avoid use during breastfeeding or consult a doctor for alternative feeding methods.
