Description
Overview of the medicine
Protide 25 mcg Inhaler is a long-acting bronchodilator used for the maintenance treatment of asthma (in combination with an inhaled corticosteroid) and for chronic obstructive pulmonary disease (COPD). It helps to relax the muscles around the airways, making it easier to breathe.
Uses & Indications
Dosage
Adults
For asthma and COPD: 25 mcg (one actuation) twice daily, approximately every 12 hours. For exercise-induced bronchospasm: 25 mcg (one actuation) at least 30 minutes before exercise.
Elderly
No dosage adjustment is generally required for elderly patients. Use adult dose with caution.
Renal_impairment
No dosage adjustment is required in patients with renal impairment.
Hepatic_impairment
Use with caution in patients with hepatic impairment. Dosage adjustment may be necessary in severe cases.
How to Take
For oral inhalation only. Shake the inhaler well before each use. Inhale deeply through the mouth while pressing down on the canister. Hold your breath for about 10 seconds, then exhale slowly. Follow specific instructions provided with the inhaler.
Mechanism of Action
Protide selectively stimulates beta-2 adrenergic receptors in bronchial smooth muscle, leading to relaxation of the smooth muscle and bronchodilation. This action helps to widen the airways and improve airflow to the lungs.
Pharmacokinetics
Onset
Bronchodilatory effect typically observed within 10-20 minutes after inhalation.
Excretion
Excreted primarily via the renal (urine) and fecal routes, with a small portion excreted as unchanged drug.
Half life
Approximately 8-12 hours in plasma.
Absorption
Rapidly absorbed after oral inhalation; systemic bioavailability is relatively low due to extensive first-pass metabolism and substantial pulmonary deposition.
Metabolism
Extensively metabolized in the liver, primarily via cytochrome P450 (CYP) enzymes.
Side Effects
Contraindications
- Acute bronchospasm or status asthmaticus (Protide is not indicated for the relief of acute bronchospasm).
- Hypersensitivity to Protide or any excipients in the formulation.
Drug Interactions
Diuretics
Co-administration with non-potassium-sparing diuretics (such as loop or thiazide diuretics) may exacerbate hypokalemia and/or ECG changes, especially at higher doses of Protide.
Beta-blockers
Concomitant use with beta-blockers, especially non-selective ones, may reduce the efficacy of Protide and potentially cause severe bronchospasm in patients with asthma. If co-administration is necessary, cardioselective beta-blockers should be used with caution.
Other Sympathomimetic Agents
Concurrent use with other sympathomimetic agents may potentiate adverse cardiovascular effects. Avoid concomitant use.
Tricyclic Antidepressants (TCAs) and Monoamine Oxidase Inhibitors (MAOIs)
These drugs can potentiate the cardiovascular effects of Protide. Use with extreme caution, if at all, or discontinue these agents well in advance of Protide administration.
Storage
Store below 30°C (86°F). Protect from direct sunlight and freezing. Do not puncture or incinerate the canister. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive beta-adrenergic stimulation (e.g., tachycardia, palpitations, tremor, headache, hypokalemia, hyperglycemia). Treatment is primarily supportive. Cardiac monitoring may be required. Protide is a long-acting agent, and the effects of an overdose may persist.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether Protide is excreted in human milk; use with caution in nursing mothers. Consult a doctor.

YesClinical Trials
Clinical trials have demonstrated Protide's efficacy in improving lung function and reducing exacerbations in patients with asthma (when used with ICS) and COPD. Ongoing studies may further evaluate its long-term safety and comparative effectiveness.
Lab Monitoring
- Routine laboratory monitoring is generally not required.
- Monitor serum potassium levels in patients predisposed to hypokalemia or those on concomitant diuretics.
Doctor Notes
- Emphasize patient education on the proper use of the inhaler and the distinction between maintenance and rescue medications.
- For asthma patients, always ensure concomitant inhaled corticosteroid therapy to mitigate the risk associated with LABA monotherapy.
- Regularly assess patient's asthma or COPD control and adjust treatment as necessary. Monitor for adverse cardiovascular effects, especially in patients with pre-existing cardiac conditions.
Patient Guidelines
- Use Protide 25 mcg Inhaler exactly as prescribed by your doctor. Do not exceed the recommended dose.
- This medicine is for maintenance treatment; it is NOT for quick relief of sudden breathing problems. Always carry your rescue inhaler for acute symptoms.
- Ensure proper inhaler technique as demonstrated by your healthcare provider to maximize drug delivery to the lungs.
- Do not stop using Protide suddenly, even if you feel better, without consulting your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next scheduled dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Protide 25 mcg Inhaler is generally not expected to affect the ability to drive or operate machinery. However, if you experience side effects such as dizziness, tremor, or headache, avoid driving or operating machinery until these symptoms subside.
Lifestyle Advice
- Avoid exposure to known asthma triggers and irritants (e.g., smoke, pollen, dust mites).
- Regular exercise can improve lung function, but ensure your asthma is well-controlled. Use Protide preventatively if advised for exercise-induced bronchospasm.
- Maintain a healthy diet and stay hydrated.
