Description
Overview of the medicine
Proxid (Pantoprazole) is a proton pump inhibitor (PPI) that decreases the amount of acid produced in the stomach. It is used to treat conditions such as acid reflux, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD and erosive esophagitis: 40 mg once daily for up to 8 weeks. For Zollinger-Ellison Syndrome: Initially 40 mg twice daily, adjusted as needed.
Elderly
No specific dose adjustment is generally required, but close monitoring is advisable.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Take orally before a meal, preferably in the morning. Swallow the tablet whole; do not chew or crush.
Mechanism of Action
Pantoprazole binds irreversibly to the H+/K+ ATPase enzyme (the proton pump) on the secretory surface of the gastric parietal cells. This inhibits the final step in gastric acid production, leading to a sustained decrease in acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 2-3 hours.
Excretion
Primarily excreted renally (approximately 80%) and via biliary/fecal route (approximately 20%).
Half life
The plasma elimination half-life is approximately 1 hour.
Absorption
Rapidly absorbed after oral administration; bioavailability is approximately 77%. Peak plasma concentrations occur 2-2.5 hours after an oral dose.
Metabolism
Extensively metabolized in the liver primarily by CYP2C19 and to a lesser extent by CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to Pantoprazole or any component of the formulation
- Concurrent use with atazanavir or nelfinavir
Drug Interactions
Warfarin
May increase INR and prothrombin time, monitor for bleeding.
Methotrexate
May increase methotrexate levels, especially with high doses.
Atazanavir/Nelfinavir
Significantly reduces absorption and efficacy of these antiretrovirals, co-administration is contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no known specific antidote for Pantoprazole overdose. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk, use with caution during lactation.
