Description
Overview of the medicine
Pulmoten is a long-acting beta2-adrenergic agonist (LABA) used for the maintenance treatment of airflow obstruction in patients with chronic obstructive pulmonary disease (COPD).
Uses & Indications
Dosage
Adults
For COPD, 75 mcg (or 150 mcg depending on brand/region) once daily via oral inhalation using the device.
Elderly
No dose adjustment required based on age.
Renal_impairment
No dose adjustment required in patients with renal impairment.
How to Take
Pulmoten capsules are for oral inhalation only and must be used with the specific inhaler device. Do not swallow the capsules.
Mechanism of Action
Indacaterol selectively stimulates beta2-adrenergic receptors in the lung, leading to relaxation of bronchial smooth muscle, resulting in bronchodilation. This effect helps to improve airflow in patients with COPD.
Pharmacokinetics
Onset
Within 5 minutes.
Excretion
Predominantly excreted via feces (94%), with a small amount (6%) excreted via urine.
Half life
Approximately 40-56 hours (terminal half-life).
Absorption
Rapidly absorbed after oral inhalation, peak plasma concentrations reached within 15-30 minutes.
Metabolism
Primarily metabolized by UGT1A1 and to a lesser extent by CYP3A4.
Side Effects
Contraindications
- Acute bronchospasm (not a rescue medication)
- Not for asthma (increased risk of asthma-related death)
- Hypersensitivity to Indacaterol or any excipients.
Drug Interactions
Diuretics
May increase risk of hypokalemia.
Beta-blockers
May reduce effect of Indacaterol.
Other sympathomimetic agents
May potentiate adverse effects.
Tricyclic antidepressants, MAO inhibitors
May potentiate cardiovascular effects.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive beta-adrenergic stimulation (e.g., tachycardia, palpitations, arrhythmia, tremor, headache, nausea, vomiting, hypokalemia, hyperglycemia). Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if Indacaterol is excreted in human milk; caution is advised.
