Overview of the medicine
Quetinil contains Quetiapine, an atypical antipsychotic used to treat schizophrenia, bipolar disorder (manic, depressive, and maintenance episodes), and as an adjunct treatment for major depressive disorder.
Uses & Indications
Dosage
Adults
Initial dose typically 25-50 mg/day, titrated up to 150-800 mg/day depending on indication and patient response. Dosing varies for IR and XR formulations.
Elderly
Lower initial doses (e.g., 25 mg/day) and slower titration recommended due to increased sensitivity.
Renal_impairment
No dosage adjustment needed for mild to severe renal impairment.
How to Take
Tablets can be taken with or without food. Extended-release tablets should be swallowed whole and not chewed or crushed.
Mechanism of Action
Quetiapine acts as an antagonist at multiple neurotransmitter receptors, including serotonin 5-HT2A and dopamine D1 and D2 receptors. It also has affinity for histamine H1 and adrenergic alpha1 and alpha2 receptors.

Onset
Onset of action varies by indication, generally within days for acute psychosis.
Excretion
Primarily excreted in urine (73%) and feces (20%), mainly as metabolites.
Half life
Approximately 6-7 hours for Quetiapine and 9-12 hours for norquetiapine (active metabolite).
Absorption
Rapidly and well absorbed after oral administration, peak plasma concentrations reached in 1.5 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, to inactive and active metabolites.
Side Effects
Contraindications

Drug Interactions
CNS depressants
May enhance sedative effects (e.g., alcohol, benzodiazepines).

May decrease Quetiapine plasma concentrations (e.g., phenytoin, carbamazepine, rifampicin).
CYP3A4 inhibitors
May increase Quetiapine plasma concentrations (e.g., ketoconazole, erythromycin).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, sedation, tachycardia, hypotension. Management is primarily supportive, maintaining airway, ventilation, and monitoring cardiovascular function.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as Quetiapine is excreted in breast milk.
