Quiet 25 mg Tablet contains Quetiapine Fumarate, an atypical antipsychotic used to treat schizophrenia, bipolar disorder, and as an adjunctive treatment for major depressive disorder. It helps restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Initial dose 25 mg twice daily, titrated upwards as tolerated to a target range of 300-800 mg/day depending on indication.
Elderly
Initial dose 25 mg once daily, then increased in increments of 25-50 mg/day to an effective dose.
Renal_impairment
No dose adjustment is generally required, but caution is advised. Initiate at 25 mg/day and titrate slowly.
How to Take
Quiet tablets should be taken orally, with or without food. For immediate-release tablets, it is usually taken twice daily. For extended-release tablets, it is taken once daily, preferably in the evening.
Mechanism of Action
Quetiapine is an atypical antipsychotic with a broad receptor binding profile. It acts as an antagonist at dopamine D1 and D2 receptors, serotonin 5-HT1A and 5-HT2A receptors, histamine H1 receptors, and adrenergic α1 and α2 receptors. Its antipsychotic action is primarily attributed to a combination of dopamine D2 and serotonin 5-HT2A receptor antagonism. It has a higher affinity for 5-HT2A receptors than for D2 receptors.

Excretion
Approximately 73% excreted in urine and 20% in feces, predominantly as metabolites. Less than 1% excreted as unchanged drug.
Half life
Approximately 6-7 hours for Quetiapine and 11-12 hours for the active metabolite norquetiapine.
Absorption
Rapidly and well absorbed after oral administration, peak plasma concentration usually attained within 1.5 hours. Bioavailability is not significantly affected by food.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4 to form norquetiapine and other inactive metabolites.
