Description
Overview of the medicine
Rabeprazole is a proton pump inhibitor (PPI) that decreases the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4-8 weeks. For Duodenal Ulcers: 20 mg once daily for 4 weeks. For H. pylori eradication: 20 mg twice daily with appropriate antibiotics for 7-14 days. For Zollinger-Ellison Syndrome: Initial dose 60 mg once daily, adjusted as needed.
Elderly
No specific dose adjustment is usually required for elderly patients, but caution should be exercised.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
Hepatic_impairment
Use with caution in patients with severe hepatic impairment; dose reduction may be considered.
How to Take
Rabeprazole capsules should be swallowed whole with water. Do not chew or crush the capsule. It is usually taken before a meal, preferably in the morning.
Mechanism of Action
Rabeprazole works by irreversibly binding to the H+/K+-ATPase enzyme (the proton pump) on the secretory surface of gastric parietal cells, thereby inhibiting the final step in gastric acid secretion.
Pharmacokinetics
Onset
Acid inhibitory effect starts within 1 hour, with maximal effect by 2-4 hours.
Excretion
Approximately 90% is excreted in the urine and 10% in the feces as metabolites.
Half life
Plasma elimination half-life is approximately 1-2 hours, but its acid-inhibitory effect lasts much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 2-4 hours. Bioavailability is approximately 52% and is not significantly affected by food.
Metabolism
Extensively metabolized in the liver via cytochrome P450 (CYP2C19 and CYP3A4) isoenzymes.
Side Effects
Contraindications
- •Hypersensitivity to rabeprazole, substituted benzimidazoles, or any component of the formulation.
- •Patients undergoing treatment with atazanavir or nelfinavir due to risk of significantly reduced antiviral efficacy.
Drug Interactions
Warfarin
Increased INR and prothrombin time.
Methotrexate
Possible increased methotrexate levels.
Atazanavir/Nelfinavir
Significantly reduced plasma concentrations of antiretrovirals, leading to loss of therapeutic effect.
Ketoconazole/Itraconazole/Digoxin
Reduced absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
There is limited experience with overdose. Symptoms may include drowsiness, tachycardia, headache, and nausea. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is unknown if rabeprazole is excreted in human milk; caution is advised when administered to a nursing woman.
