Description
Overview of the medicine
Rabeprazole is a proton pump inhibitor (PPI) used to treat conditions caused by excess stomach acid, such as heartburn, acid reflux, and ulcers. It works by reducing the amount of acid produced in the stomach.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4-8 weeks. For duodenal ulcer: 20 mg once daily for 4 weeks. For H. pylori eradication: 20 mg twice daily for 7 days (in combination therapy).
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Usually taken once daily in the morning before a meal. Swallow the tablet whole; do not chew or crush.
Mechanism of Action
Rabeprazole inhibits the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cells. This irreversibly blocks the final step of acid production, thereby reducing gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour, full effect within 2-3 hours.
Excretion
Primarily excreted via the urine (90%) as metabolites, with a small portion excreted in feces.
Half life
Approximately 1-2 hours (plasma half-life), but duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 2-5 hours. Bioavailability is about 52% and does not increase with multiple doses.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications

Drug Interactions
Digoxin
Increased digoxin exposure.
Warfarin
Increased INR/prothrombin time; monitor closely.
Methotrexate
Possible increased plasma levels of methotrexate.
Ketoconazole, Itraconazole, Atazanavir
Decreased absorption of drugs that depend on gastric pH for absorption.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Symptomatic and supportive treatment. Rabeprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy category B. Use only if clearly needed. Excreted in animal milk; advise caution during breastfeeding.
