Description
Overview of the medicine
Raciprox 250 mg suspension contains Ciprofloxacin, a broad-spectrum fluoroquinolone antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
250 mg to 750 mg orally twice daily, depending on the infection type and severity. For suspension, dosage is adjusted by volume.
Elderly
No specific dose adjustment for elderly patients with normal renal function. Adjust for renal impairment if present.
Renal_impairment
Dosage reduction is necessary for patients with creatinine clearance <30 mL/min. e.g., 250-500 mg once daily.
How to Take
Shake the suspension well before each use. Can be taken with or without food. Avoid taking with dairy products or calcium-fortified juices alone, as absorption may be reduced. Maintain adequate hydration.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 1-2 hours.
Excretion
Primarily excreted renally (50-70% as unchanged drug), with some fecal excretion (15%).
Half life
Approximately 3-5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract. Oral bioavailability is approximately 70-80%.
Metabolism
Partially metabolized in the liver (10-20%) to less active metabolites.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones
- Concomitant use with tizanidine
Drug Interactions
Warfarin
Potentiation of anticoagulant effect, requiring INR monitoring.
Tizanidine
Increased risk of hypotension and sedation.
Theophylline
Increased plasma levels of theophylline, leading to toxicity.
Corticosteroids
Increased risk of tendinopathy and tendon rupture.
Antacids (containing aluminum, magnesium)
Reduced absorption of ciprofloxacin. Administer ciprofloxacin at least 2 hours before or 6 hours after antacids.
Storage
Store dry powder below 30°C, protected from light and moisture. After reconstitution, store at room temperature (15-30°C) or refrigerated (2-8°C) and discard unused portion after 14 days. Do not freeze.
Overdose
Limited data on overdose. Symptoms may include renal toxicity, crystalluria, CNS effects (e.g., confusion, tremor, seizures). Management includes gastric lavage, activated charcoal, and symptomatic and supportive care. Ensure adequate hydration.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Ciprofloxacin is excreted in breast milk; therefore, caution should be exercised when administered to a nursing woman.
