Description
Overview of the medicine
Ramdivir is an antiviral medication used for the treatment of coronavirus disease 2019 (COVID-19) in hospitalized patients. It helps to reduce the viral load and shorten recovery time.
Uses & Indications
Dosage
Adults
200 mg loading dose on Day 1, followed by 100 mg once daily for 4 subsequent days (total 5 days) via intravenous infusion. For severe cases, treatment may extend to 10 days.
Elderly
No specific dose adjustment is required, but caution is advised due to potential for decreased renal/hepatic function.
Renal_impairment
Not recommended for patients with eGFR <30 mL/min due to potential accumulation of excipient sulfobutylether beta-cyclodextrin sodium (SBECD).
How to Take
Administer via intravenous (IV) infusion over 30 to 120 minutes. It must be diluted before administration. Do not administer as a rapid IV injection.
Mechanism of Action
Ramdivir is a prodrug that is metabolized intracellularly to its active nucleoside triphosphate analog. This analog then interferes with viral RNA-dependent RNA polymerase, thereby inhibiting viral replication.
Pharmacokinetics
Onset
Rapid, peak plasma concentration of active metabolite reached in approximately 1.5 hours.
Excretion
Primarily renal excretion (approximately 70% as metabolites, 10% unchanged drug in urine).
Half life
Parent drug: ~1 hour; Active metabolite (GS-441524): ~20-25 hours (intracellular).
Absorption
Administered intravenously, hence 100% bioavailability.
Metabolism
Rapidly metabolized via ester hydrolysis to form the parent nucleoside metabolite GS-441524, which is then phosphorylated to the active triphosphate.
Side Effects
Contraindications
- History of hypersensitivity reaction to Ramdivir or any components of the formulation.
Drug Interactions
Certain antivirals
Potential for additive toxicity or reduced efficacy; consult official guidelines for specific interactions.
Chloroquine/Hydroxychloroquine
Coadministration with Ramdivir is not recommended as it may reduce antiviral activity.
Storage
Store below 30°C (86°F), protect from light. Reconstituted solution should be used immediately or stored for a limited time.
Overdose
No specific antidote available. In case of overdose, manage with supportive care and monitor vital signs and organ function.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Caution advised during breastfeeding; evaluate risk-benefit.
