Description
Overview of the medicine
Ramicol-125-mg-suspension is an antibiotic used to treat a wide range of bacterial infections. It contains Amoxicillin, which belongs to the penicillin class of antibiotics and works by stopping the growth of bacteria. It is commonly prescribed for respiratory tract infections, urinary tract infections, skin infections, and certain ear, nose, and throat infections, especially in children.
Uses & Indications
Dosage
Adults
Not typically used as a 125mg/5ml suspension for adults. Adult dosage of Amoxicillin is usually 250-500 mg every 8 hours, or 875 mg every 12 hours.
Elderly
Similar to adult dosage, but caution with renal impairment. Dosage adjustment required based on creatinine clearance.
Renal_impairment
Dosage adjustment required based on creatinine clearance. For CrCl < 30 mL/min, dosage intervals should be extended.
How to Take
Oral. Administer with or without food. Shake well before use. Use a measuring spoon or oral syringe for accurate dosing. For children, ensure the full prescribed course is completed.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Within 1-2 hours.
Excretion
Primarily excreted unchanged by the kidneys via tubular secretion and glomerular filtration.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed after oral administration, bioavailability is approximately 70-80%. Peak plasma concentrations occur within 1-2 hours.
Metabolism
Partially metabolized in the liver to inactive metabolites; approximately 10-25%.
Side Effects
Contraindications
- Known hypersensitivity to penicillin or other beta-lactam antibiotics
- History of Amoxicillin-associated cholestatic jaundice/hepatic dysfunction
- Infectious mononucleosis (increases risk of maculopapular rash)
Drug Interactions
Probenecid
Increases amoxicillin levels by decreasing renal tubular secretion, potentially leading to higher and more prolonged plasma concentrations.
Methotrexate
Increases methotrexate toxicity due to reduced renal clearance.
Oral Contraceptives
May reduce efficacy of oral contraceptives (rarely significant, but advise additional contraception).
Warfarin and other oral anticoagulants
May prolong prothrombin time; monitor INR closely, especially when initiating or discontinuing Amoxicillin.
Storage
Store dry powder below 30°C in a dry place, away from direct light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and do not freeze.
Overdose
Symptoms include gastrointestinal disturbances (nausea, vomiting, diarrhea) and fluid/electrolyte imbalance. Renal impairment may lead to crystalluria. Treatment is symptomatic and supportive. Hemodialysis can effectively remove amoxicillin from circulation in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe in pregnancy, but should be used only if clearly needed. Excreted in breast milk in small amounts; generally considered safe during lactation, but monitor infant for potential adverse effects like diarrhea, candidiasis, or rash.
