Description
Overview of the medicine
Ranzo (Rabeprazole) is a proton pump inhibitor that decreases the amount of acid produced in the stomach. It is used to treat conditions like heartburn, acid reflux, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Gastroesophageal Reflux Disease (GERD): 20 mg once daily for 4 to 8 weeks. Duodenal Ulcers: 20 mg once daily for 4 weeks. H. pylori eradication: 20 mg twice daily in combination with antibiotics.
Elderly
No specific dose adjustment is required.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take orally, usually once daily in the morning before a meal. Swallow the tablet whole; do not crush, chew, or split it.
Mechanism of Action
Rabeprazole irreversibly blocks the hydrogen-potassium adenosine triphosphatase (H+/K+-ATPase) enzyme system, also known as the proton pump, in the gastric parietal cells. This reduces gastric acid secretion.
Pharmacokinetics
Onset
Onset of antisecretory effect within 1 hour.
Excretion
Approximately 90% is excreted in the urine and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 1 to 2 hours.
Absorption
Rapidly absorbed after oral administration. Oral bioavailability is about 52%.
Metabolism
Metabolized primarily in the liver by the cytochrome P450 system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Known hypersensitivity to rabeprazole, substituted benzimidazoles, or any component of the formulation.
- Patients with severe liver impairment.
Drug Interactions
Digoxin
Increased digoxin levels.
Warfarin
Increased INR and prothrombin time.
Methotrexate
Increased methotrexate levels, especially with high dose methotrexate.
Atazanavir/Nelfinavir
Reduced plasma levels, potentially reducing efficacy. Co-administration generally not recommended.
Ketoconazole/Itraconazole
Decreased absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no known specific antidote. In case of overdose, treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is not known whether rabeprazole is excreted in human milk, therefore caution should be exercised when administered to a nursing mother.
