Description
Overview of the medicine
Rasagiline 0.5 mg tablet is an irreversible selective monoamine oxidase-B (MAO-B) inhibitor used in the treatment of Parkinson's disease. It helps to improve motor symptoms and may be used as monotherapy or as an adjunct to levodopa.
Uses & Indications
Dosage
Adults
0.5 mg once daily, with or without levodopa. Maximum recommended dose is 1 mg once daily.
Elderly
No specific dosage adjustment required based on age.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Administer orally once daily, with or without food. Avoid taking with foods rich in tyramine.
Mechanism of Action
Rasagiline selectively and irreversibly inhibits monoamine oxidase-B (MAO-B), an enzyme responsible for the breakdown of dopamine in the brain. By inhibiting MAO-B, rasagiline increases the levels of dopamine in the striatum, which helps to alleviate the motor symptoms of Parkinson's disease.
Pharmacokinetics
Onset
Clinical effects may be observed within a few weeks.
Excretion
Mainly excreted in urine (approximately 62%) and feces (approximately 22%) as metabolites.
Half life
Approximately 0.6-2 hours (parent drug), but the irreversible inhibition means effects last longer.
Absorption
Rapidly absorbed, reaching peak plasma concentrations within approximately 1 hour. Bioavailability is about 36%.
Metabolism
Primarily metabolized in the liver by CYP1A2 to 1(R)-aminoindan, which is inactive. Not metabolized via other major CYP isoenzymes.
Side Effects
Contraindications
- Concomitant use with pethidine or other opioid analgesics
- Concomitant use with other MAO inhibitors (including herbal products like St. John's Wort)
- Concomitant use with sympathomimetic agents (e.g., decongestants, amphetamines)
- Concomitant use with cyclobenzaprine, dextromethorphan, or tramadol
- Severe hepatic impairment
Drug Interactions
Ciprofloxacin
Increases plasma levels of rasagiline due to CYP1A2 inhibition.
SSRIs, SNRIs, TCAs
Risk of serotonin syndrome.
Other MAO inhibitors (e.g., phenelzine, tranylcypromine, selegiline)
Risk of hypertensive crisis and serotonin syndrome.
Sympathomimetic agents (e.g., pseudoephedrine, ephedrine, amphetamines)
Risk of hypertensive crisis.
Pethidine, Tramadol, Meperidine, Dextromethorphan, Methadone, Propoxyphene
Risk of serotonin syndrome (agitation, confusion, hallucinations, hyperthermia, tremor, myoclonus, hyperreflexia, incoordination, severe diarrhea).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe hypertension, serotonin syndrome, or cardiovascular effects. Treatment is symptomatic and supportive, including monitoring vital signs and managing hypertension.
Pregnancy & Lactation

Lab Monitoring
- Monitor blood pressure regularly, especially at initiation and dose escalation.
- Monitor for signs and symptoms of serotonin syndrome.
Doctor Notes
- Emphasize adherence to dietary restrictions for tyramine, especially if dose exceeds 0.5 mg/day or if patient is taking other MAOIs.
- Screen patients for impulse control disorders regularly.
- Monitor for emergent skin lesions due to potential melanoma risk.

- Take as directed by your doctor, usually once daily.
- Do not stop taking rasagiline abruptly without consulting your doctor.
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
