Description
Overview of the medicine
Rasagiline is a medicine used to treat Parkinson's disease. It belongs to a class of drugs called monoamine oxidase-B (MAO-B) inhibitors. It helps improve symptoms of Parkinson's by increasing the levels of dopamine in the brain.
Uses & Indications
Dosage
Adults
The recommended dose is 1 mg once daily, taken orally with or without food.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. Caution is advised in severe renal impairment.
How to Take
Take the tablet orally once daily, with or without food. It should be swallowed whole with water and not crushed or chewed.
Mechanism of Action
Rasagiline selectively and irreversibly inhibits monoamine oxidase-B (MAO-B), an enzyme responsible for the breakdown of dopamine in the brain. This inhibition leads to increased and sustained levels of dopamine in the brain, thereby improving motor symptoms associated with Parkinson's disease.
Pharmacokinetics
Onset
Clinical effects may be observed within a few weeks of initiation.
Excretion
Approximately 62% of a dose is excreted in the urine and 21% in the feces. Less than 1% is excreted unchanged in the urine.
Half life
The elimination half-life is approximately 1-3 hours for the parent compound, but the pharmacological effect is longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, reaching peak plasma concentrations within approximately 0.5-1 hour.
Metabolism
Extensively metabolized in the liver, primarily by CYP1A2, to its major active metabolite, 1(R)-aminoindan. Subsequent metabolism occurs via glucuronidation and hydroxylation.
Side Effects
Contraindications
- Concomitant use with pethidine or other opioid analgesics, tramadol, methadone, propoxyphene.
- Concomitant use with other monoamine oxidase (MAO) inhibitors (including other MAO-B inhibitors, e.g., selegiline) or within 14 days of discontinuing such treatments.
- Concomitant use with sympathomimetic amines (e.g., in decongestants or cold remedies).
- Concomitant use with selective serotonin reuptake inhibitors (SSRIs), serotonin-norepinephrine reuptake inhibitors (SNRIs), tricyclic antidepressants (TCAs), or cyclobenzaprine.
- Severe hepatic impairment.
Drug Interactions
Ciprofloxacin
Can increase rasagiline levels due to CYP1A2 inhibition, requiring caution.
Dextromethorphan
Potential for psychotic episodes or other adverse reactions.
SSRIs, SNRIs, TCAs, Cyclobenzaprine
Increased risk of serotonin syndrome.
Pethidine, Tramadol, Methadone, Propoxyphene
Risk of serious adverse reactions, including serotonin syndrome.
Sympathomimetic amines (e.g., decongestants)
Risk of hypertensive reactions.
Other MAO Inhibitors (including non-selective MAOIs)
Increased risk of hypertensive crisis or serotonin syndrome.
Storage
Store at room temperature (below 25°C), away from moisture and light. Keep out of reach of children.
Overdose
Symptoms of overdose may include hypertension, orthostatic hypotension, postural hypotension, severe dyskinesia, agitation, dizziness, somnolence, and serotonin syndrome. Treatment should be supportive and symptomatic. Gastric lavage and activated charcoal may be considered in acute overdose.
Pregnancy & Lactation
Pregnancy Category C. Studies in animals have shown adverse effects on the fetus; there are no adequate and well-controlled studies in pregnant women. Rasagiline is not recommended during pregnancy. It is unknown whether rasagiline is excreted in human milk, thus, caution should be exercised when administered to a nursing woman.
