Overview of the medicine
Rasonix 20 mg Capsule contains Esomeprazole, a proton pump inhibitor, used to reduce stomach acid production. It treats conditions like acid reflux (GERD), heartburn, ulcers, and prevents NSAID-induced gastric damage.
Uses & Indications
Dosage
Adults
For GERD: 20 mg or 40 mg once daily for 4-8 weeks. For Erosive Esophagitis: 20 mg or 40 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg twice daily (in combination with antibiotics).
Elderly
No specific dosage adjustment is generally required, but caution is advised in very elderly patients due to potential comorbidities.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function. However, caution is advised in patients with severe renal impairment.
How to Take
Take Rasonix capsule whole at least one hour before a meal. Do not chew or crush the capsule. If swallowing is difficult, the capsule can be opened and the pellets mixed with a small amount of water or soft food (e.g., apple sauce), then swallowed immediately.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cell. This leads to a profound and sustained reduction in gastric acid secretion.
Pharmacokinetics
Onset
Acid inhibitory effect starts within one hour of oral administration.
Excretion
Approximately 80% of metabolites are excreted in the urine, and 20% in the feces.
Half life
Plasma elimination half-life is approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% with a 20 mg dose, increasing to 89% with repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an antiretroviral).
Drug Interactions
Warfarin
Increased INR (International Normalized Ratio) and prothrombin time, monitor for bleeding.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase serum levels of methotrexate, particularly with high doses.
Nelfinavir/Atazanavir
Reduced plasma concentrations of antiretrovirals, avoid concomitant use.
