Description
Overview of the medicine
Doxazosin Mesylate Extended Release is an alpha-1 adrenergic blocker used to treat high blood pressure (hypertension) and symptoms of benign prostatic hyperplasia (BPH). It works by relaxing blood vessels and muscles in the prostate and bladder neck. Note: 25 mg is a very high dose for doxazosin ER and is not commonly available in standard therapeutic ranges.
Uses & Indications
Dosage
Adults
For hypertension, typical starting dose is 1 mg ER once daily, titrating up to a maximum of 4-8 mg ER once daily. For BPH, typical starting dose is 4 mg ER once daily, titrating up to a maximum of 8 mg ER once daily. A 25 mg ER dose is highly unusual and not part of standard guidelines for Doxazosin. If prescribed, it would be for very specific, resistant cases under strict medical supervision.
Elderly
Similar to adults, start with the lowest effective dose.
Renal_impairment
No dosage adjustment usually required, but use with caution.
How to Take
Take orally once daily, in the morning, with or without food. Swallow the tablet whole; do not chew, crush, or divide.
Mechanism of Action
Selectively blocks alpha-1 adrenergic receptors in vascular smooth muscle, leading to vasodilation and reduced peripheral vascular resistance. In BPH, it relaxes the smooth muscle in the prostate and bladder neck, improving urine flow.
Pharmacokinetics
Onset
BP reduction: full effect over 1-2 weeks. BPH symptom improvement: several weeks.
Excretion
Primarily fecal (63-65%), minor renal excretion (9-10%), mainly as metabolites.
Half life
Approximately 22 hours.
Absorption
Well absorbed orally, sustained release with ER formulation. Peak plasma concentration 8-9 hours. Bioavailability 62-69%.
Metabolism
Extensive hepatic metabolism, primarily O-demethylation and hydroxylation.
Side Effects
Contraindications
- Hypersensitivity to doxazosin, other quinazolines (e.g., prazosin, terazosin), or any component of the formulation.
- History of orthostatic hypotension with alpha-blockers.
- Pre-existing gastrointestinal narrowing or difficulty swallowing (for ER formulations).
Drug Interactions
NSAIDs
May reduce the antihypertensive effect.
PDE5 inhibitors (e.g., sildenafil, tadalafil)
Concomitant use can lead to symptomatic hypotension.
Other antihypertensives (e.g., beta-blockers, diuretics)
Additive hypotensive effects.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms typically involve marked hypotension. Management includes placing the patient in a supine position, administrating intravenous fluids, and if necessary, vasopressors.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Doxazosin is excreted in small amounts in human milk; caution is advised when administered to a nursing woman.

YesClinical Trials
Doxazosin has been extensively studied in clinical trials for hypertension and BPH, demonstrating efficacy and safety. Ongoing post-marketing surveillance and studies explore long-term outcomes and specific patient populations.
Lab Monitoring
- Regular blood pressure monitoring, especially for orthostatic changes.
- Assessment of BPH symptoms (e.g., International Prostate Symptom Score, IPSS).
Doctor Notes
- Counsel patients on the risk of first-dose hypotension and orthostatic effects, especially when initiating treatment or increasing dosage.
- Advise patients to notify ophthalmologists about doxazosin use prior to cataract surgery.
- The 25 mg strength of Doxazosin ER is highly unusual and outside the standard therapeutic range for both hypertension and BPH. Verify the prescription and consider potential reasons for such a high dose, or if it refers to a different active ingredient or formulation.
Patient Guidelines
- Take the medicine at the same time each day.
- Avoid sudden changes in body position (e.g., standing up quickly) to minimize dizziness.
- Inform your doctor if you plan to have cataract surgery, as Doxazosin can cause Intraoperative Floppy Iris Syndrome (IFIS).
- Do not stop taking the medicine without consulting your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as remembered unless it is almost time for the next scheduled dose. Do not take a double dose to make up for a missed one.
Driving Precautions
May cause dizziness or drowsiness, especially at the start of treatment or when changing doses. Avoid driving or operating machinery until you know how you react to the medication.
Lifestyle Advice
- Maintain a healthy lifestyle, including a balanced diet and regular exercise.
- Limit alcohol intake, as it can worsen hypotensive effects.
- Avoid activities that require mental alertness until you know how the medicine affects you.
