Description
Overview of the medicine
Razosin-ER 5 mg Tablet contains Terazosin, an alpha-1 blocker. It is used to treat benign prostatic hyperplasia (BPH) and high blood pressure (hypertension). It works by relaxing the muscles in the prostate and bladder neck, and by widening blood vessels.
Uses & Indications
Dosage
Adults
BPH: Initially 1 mg once daily at bedtime, increasing gradually to a maximum of 5 mg or 10 mg once daily. Hypertension: Initially 1 mg once daily at bedtime, titrated up to 1-5 mg daily based on patient response.
Elderly
Same as adults, but initiate with caution and monitor for orthostatic hypotension due to increased sensitivity.
Renal_impairment
No dosage adjustment is usually necessary, but caution is advised in severe renal impairment.
How to Take
Take orally, once daily, preferably at bedtime, to minimize the effects of orthostatic hypotension. Swallow the tablet whole; do not crush, chew, or divide.
Mechanism of Action
Terazosin is a selective alpha-1 adrenergic receptor antagonist. It blocks alpha-1 receptors located in the smooth muscle of the prostate, bladder neck, and blood vessels. This relaxation of smooth muscle in the prostate and bladder neck improves urine flow in BPH patients. In blood vessels, it causes vasodilation, leading to a reduction in blood pressure.
Pharmacokinetics
Onset
Blood pressure lowering effect may be observed within a few hours. Symptomatic improvement in BPH may take 2-4 weeks.
Excretion
Approximately 40% excreted in urine and 60% in feces, largely as metabolites.
Half life
Approximately 8-12 hours.
Absorption
Well absorbed after oral administration, bioavailability is approximately 90%. Absorption is not significantly affected by food.
Metabolism
Extensively metabolized in the liver, primarily by O-demethylation, with several inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to terazosin or other quinazoline derivatives (e.g., prazosin, doxazosin).
- History of orthostatic hypotension or syncope after taking similar medications.
Drug Interactions
Other antihypertensives
Concurrent use may result in additive hypotensive effects. Monitor blood pressure closely.
PDE5 inhibitors (e.g., sildenafil, tadalafil)
Increased risk of symptomatic hypotension. Administer with caution and consider lower initial doses of both drugs.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose typically involve pronounced hypotension. Management should include cardiovascular support (e.g., elevating legs, administering intravenous fluids) and vasopressors if necessary. Dialysis is unlikely to be beneficial.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether terazosin is excreted in human milk; use with caution in nursing mothers.

Clinical Trials
Terazosin has been extensively studied in various clinical trials demonstrating its efficacy and safety in the treatment of BPH and hypertension. Key trials have evaluated its long-term use and comparison with other alpha-blockers.
Lab Monitoring
- Blood pressure monitoring, especially during initial therapy and dose titration. Periodic prostate-specific antigen (PSA) levels for BPH patients.
Doctor Notes
- Counsel patients on the 'first-dose effect' of orthostatic hypotension and advise taking the initial dose at bedtime.
- Titrate dosage slowly to minimize side effects and optimize therapeutic response.
- Monitor blood pressure regularly, particularly when initiating or adjusting therapy.
Patient Guidelines
- Take your medication at bedtime to minimize dizziness or fainting spells.
- Stand up slowly from a sitting or lying position to avoid sudden drops in blood pressure.
- Inform your doctor if you experience severe dizziness, fainting, or prolonged erection.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
This medication may cause dizziness, drowsiness, or fainting, especially at the beginning of treatment or after a dose increase. Avoid driving or operating heavy machinery until you are certain you can perform such activities safely.
Lifestyle Advice
- Maintain a healthy diet and regular exercise to support cardiovascular health.
- Avoid activities requiring mental alertness, such as driving or operating machinery, until you know how this medicine affects you.
