Description
Overview of the medicine
Reflacid contains Esomeprazole, a proton pump inhibitor (PPI) used to treat conditions where there is too much acid in the stomach.
Uses & Indications
Dosage
Adults
For most conditions, 20-40 mg once daily, 30-60 minutes before a meal. Duration varies by indication.
Elderly
No specific dose adjustment is generally required for elderly patients.
Renal_impairment
No dose adjustment is generally required for patients with renal impairment.
How to Take
Take the capsule orally, at least one hour before a meal. Swallow whole with water; do not chew or crush. For patients with difficulty swallowing, the capsule can be opened and the pellets mixed with a small amount of water or applesauce.
Mechanism of Action
Esomeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby reducing gastric acid secretion.

Excreted primarily in urine (80%) and feces (20%).

Absorption
Rapidly absorbed after oral administration. Bioavailability increases with repeated dosing.

Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased digoxin levels.
Warfarin
Increased INR and prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Atazanavir, Nelfinavir
Reduced plasma levels of these antiretrovirals, potentially leading to loss of therapeutic effect and development of resistance.
Ketoconazole, Itraconazole
Reduced absorption due to increased gastric pH.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Limited experience with overdose. Symptoms may include gastrointestinal disturbances and weakness. Treatment is symptomatic and supportive.

Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
