Overview of the medicine
Reladol is a brand of Paracetamol, a widely used medication for the relief of pain and reduction of fever. It works by inhibiting prostaglandin synthesis primarily in the central nervous system.
Uses & Indications
Dosage
Adults
For 500 mg tablet: 1-2 tablets (500-1000 mg) every 4-6 hours as needed. Maximum 4000 mg in 24 hours.
Elderly
No specific dose reduction required, but caution is advised in frail elderly patients or those with impaired liver/kidney function.
Renal_impairment
Dose interval may need to be extended in severe renal impairment (e.g., creatinine clearance <10 mL/min, dose every 8 hours).
How to Take
Oral (tablets, syrup, suspension, drops) or Rectal (suppositories). Can be taken with or without food. For rapid absorption, take on an empty stomach.
Mechanism of Action
Paracetamol primarily acts by inhibiting prostaglandin synthesis within the central nervous system, which contributes to its analgesic (pain-relieving) and antipyretic (fever-reducing) effects. Its exact mechanism on COX enzymes is not fully understood, but it is believed to selectively inhibit COX-2 and also has an effect on COX-3.

Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Approximately 1.25 to 3 hours in adults.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation. A small amount is metabolized by cytochrome P450 to a highly reactive intermediate (NAPQI) which is detoxified by glutathione.

Alcohol
Increased risk of hepatotoxicity with chronic alcohol consumption.
Warfarin and other coumarins
May increase the anticoagulant effect, leading to an increased risk of bleeding, especially with prolonged use of high doses of paracetamol.
Carbamazepine, Phenobarbital, Phenytoin, Rifampicin
Increased risk of hepatotoxicity due to induction of hepatic enzymes.
