Description
Overview of the medicine
Relsin 4 mg Capsule is an anxiolytic medication used to treat anxiety disorders and insomnia. It helps to calm the brain and nerves.
Uses & Indications
Dosage
Adults
For anxiety: 4 mg once daily at bedtime, or 2 mg twice daily. Maximum dose: 8 mg per day. For insomnia: 4 mg once daily at bedtime.
Elderly
Initiate with 2 mg once daily at bedtime; dosage adjustment may be needed based on response and tolerability.
Renal_impairment
No specific dose adjustment for mild to moderate renal impairment. For severe impairment, consider starting with 2 mg once daily and monitor closely.
How to Take
Take the capsule orally with or without food. Swallow whole, do not chew or crush. For insomnia, take shortly before bedtime.
Mechanism of Action
Relsin selectively modulates GABA-A receptors, leading to increased chloride ion influx, which hyperpolarizes neurons and reduces neuronal excitability, resulting in anxiolytic, sedative, and hypnotic effects.
Pharmacokinetics
Onset
Anxiolytic and sedative effects typically begin within 30-60 minutes after oral administration.
Excretion
Excreted mainly through the urine as metabolites, with a small amount excreted in feces.
Half life
The elimination half-life ranges from 6 to 8 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations reached within 1-2 hours. Oral bioavailability is approximately 80%.
Metabolism
Primarily metabolized in the liver via cytochrome P450 (CYP) enzymes, particularly CYP3A4 and CYP2C19, into inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Relsin or any component of the formulation
- •Severe respiratory insufficiency
- •Sleep apnea syndrome
- •Severe hepatic impairment
- •Myasthenia gravis
Drug Interactions
Rifampicin (CYP Inducer)
May decrease Relsin plasma concentrations, reducing its efficacy.
CYP2C19 Inhibitors (e.g., omeprazole, fluoxetine)
May also increase Relsin plasma concentrations.
CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin)
May increase Relsin plasma concentrations, leading to enhanced effects and potential for adverse reactions.
CNS Depressants (e.g., alcohol, opioids, other sedatives)
Concomitant use may potentiate CNS depressant effects, leading to increased sedation, respiratory depression, and coma.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive drowsiness, confusion, ataxia, slurred speech, hypotension, and respiratory depression. In severe cases, coma and death may occur. Treatment involves supportive care, maintaining a patent airway, gastric lavage if recent ingestion, and close monitoring of vital signs. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy unless the potential benefits outweigh the potential risks to the fetus. Small amounts may be excreted in breast milk; therefore, it is not recommended for nursing mothers. Consult your doctor.
