Overview of the medicine
Relsin 8 mg Capsule contains Silodosin, an alpha-1 adrenergic antagonist, used for the treatment of signs and symptoms of benign prostatic hyperplasia (BPH). It works by relaxing the muscles in the prostate and bladder neck, making it easier to urinate.
Uses & Indications
Dosage
Adults
8 mg once daily with a meal.
Elderly
No dosage adjustment is required for elderly patients with normal renal function.
Renal_impairment
Moderate renal impairment (CrCl 30-50 mL/min): 4 mg once daily. Severe renal impairment (CrCl < 30 mL/min): Not recommended.
How to Take
Take orally, once daily with a meal. Swallow the capsule whole; do not crush, chew, or open it.
Mechanism of Action
Silodosin selectively antagonizes alpha-1A adrenergic receptors, which are primarily located in the human prostate, bladder base, bladder neck, prostatic capsule, and prostatic urethra. Blockade of these alpha-1A adrenergic receptors causes the smooth muscle in these tissues to relax, resulting in an improvement in urine flow and a reduction in BPH symptoms.

Onset

Excretion
Excreted primarily in urine (approximately 33%) and feces (approximately 55%).
Half life
Approximately 13 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 2.5-4.5 hours.
Metabolism
Primarily metabolized in the liver by CYP3A4, glucuronidation, and alcohol dehydrogenase.
Side Effects
Contraindications
- •Hypersensitivity to Silodosin or any component of the formulation.
- •Severe hepatic impairment.
- •Severe renal impairment (CrCl < 30 mL/min).
- •Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin).
Drug Interactions
Digoxin
Silodosin is a P-glycoprotein substrate; co-administration with digoxin (a P-glycoprotein substrate) may affect digoxin levels. Monitor closely.
Antihypertensives
Increased risk of hypotension when used with other antihypertensive medications.
Other Alpha-Adrenergic Blocking Agents
Potential for additive hypotensive effects; concomitant use is not recommended.
PDE5 Inhibitors (e.g., sildenafil, tadalafil)
Risk of symptomatic orthostatic hypotension due to additive vasodilatory effects. Use with caution.
Strong CYP3A4 Inhibitors (e.g., ketoconazole, clarithromycin)
Can significantly increase silodosin exposure, increasing risk of side effects like orthostatic hypotension. Concomitant use is contraindicated.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to severe orthostatic hypotension and compensatory tachycardia. Treatment should involve supportive measures, including keeping the patient supine, fluid resuscitation, and vasopressors if necessary. Monitor blood pressure and heart rate.
