Description
Overview of the medicine
Remacid contains Ranitidine, an H2-receptor antagonist that inhibits stomach acid production. It is used to treat and prevent various conditions caused by excessive stomach acid.
Uses & Indications
Dosage
Adults
For active duodenal or gastric ulcers: 150 mg twice daily or 300 mg once daily at bedtime. For GERD: 150 mg twice daily.
Elderly
Generally no dosage adjustment required, but renal function should be assessed.
Renal_impairment
For creatinine clearance <50 mL/min, reduce dose to 150 mg once daily.
How to Take
Tablets can be taken orally, with or without food. Injections are administered intravenously or intramuscularly.
Mechanism of Action
Ranitidine selectively blocks histamine H2-receptors on the gastric parietal cells, thereby reducing both basal and stimulated gastric acid secretion.
Pharmacokinetics
Onset
Within 30-60 minutes after oral administration.
Excretion
Mainly through urine (60-70% as unchanged drug) and some in feces.
Half life
Approximately 2-3 hours, prolonged in renal impairment.
Absorption
Rapidly absorbed after oral administration, bioavailability approximately 50%. Peak plasma concentrations are reached within 1-3 hours.
Metabolism
Primarily in the liver, producing N-oxide, S-oxide, and demethylated metabolites.
Side Effects
Contraindications
- Hypersensitivity to ranitidine or any component of the formulation
- Acute porphyria
Drug Interactions
Warfarin
Monitor INR carefully, as ranitidine may prolong prothrombin time.
Atazanavir
Reduced plasma levels of atazanavir; co-administration is generally not recommended.
Ketoconazole, Itraconazole
Reduced absorption due to ranitidine's effect on gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include transient adverse effects. Management involves symptomatic and supportive treatment. Gastric lavage and activated charcoal may be considered if recent ingestion.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk, so caution advised during lactation.
